临床药师主导的管理模式对老年高血压患者治疗效果的影响

• 【摘要】目的    评估临床药师主导的管理模式对老年高血压患者治疗效果的影响。方法    选取2016年 8月至2017年11月就诊于首都医科大学附属北京同仁医院的老年高血压患者80例，通过计算机产生随机数进行分组，分为干预组（41例）和对照组（39例）。干预组患者予以8次针对生活方式和用药依从性的健康教育、住院教育或电话随访；对照组患者仅予2次健康教育。比较出院3个月后2组患者治疗效果的差异。结果    70例患者（87.5%）完成3个月随访，其中干预组35例，对照组35例。出院3个月时，干预组血压达标率明显高于对照组［100.0%（35/35）比80.0%（28/35）］，吸烟率明显低于对照组［2.9%（1/35）比20.0%（7/35）］，药物了解程度评分明显优于对照组［（1.9±0.3）分比（2.3±0.5）分］，用药依从性评分明显优于对照组［（1.8±0.5）分比（2.4±0.5）分］，差异均有统计学意义（均P＜0.05）。结论    临床药师主导的管理模式可明显提高老年高血压患者血压达标率及用药依从性。

• 【Abstract】Objective    To evaluate the effect of management model guided by clinical pharmacists on elderly patients with hypertension. Methods    From August 2016 to November 2017, 80 elderly patients with hypertension in Beijing Tongren Hospital, Capital Medical University were randomly divided into intervention group（n＝41）and control group（n＝39）. The intervention group had 8 rounds of health education focusing on healthy lifestyle and medication adherence during hospitalization and follow-up. The control group only had 2 times of health education. Antihypertensive therapeutic effect was analyzed 3 months after discharge. Results    Seventy patients（87.5%） completed 3-month follow-up, with 35 cases in each group. Standard rate of blood pressure in the intervention group was higher than that in the control group［100.0%（35/35） vs 80.0%（28/35）］（P＜0.05）. Smoking rate in the intervention group was lower than that in the control group［2.9%（1/35） vs 20.0%（7/35）］（P＜0.05）. Awareness score of antihypertensive agents in the intervention group was higher than that in the control group［（1.9±0.3） vs （2.3±0.5）］（P＜0.05）. Medication compliance score in the intervention group was higher than that in the control group［（1.8±0.5） vs （2.4±0.5）］（P＜0.05）. Conclusion    Clinical pharmacists-guided disease management model can help improve the standard rate of blood pressure and the medication compliance in elderly patients with hypertension.
