【摘要】目的 探讨香连片灌服对反流性食管炎(RE)大鼠食管黏膜菌群构成的影响。方法 采用“幽门部分结扎及前胃完全结扎手术”建立大鼠RE模型;24只健康雄性SD大鼠用随机数字表法分为假手术组、蒸馏水组(RE对照组)、香连片组(RE观察组),每组8只。术后1周后香连片组每日灌服香连片蒸馏水溶液,假手术组、蒸馏水组每日给予相同容量蒸馏水灌胃,2周后获取2块食管下段黏膜组织,1块用于提取黏膜样本细菌基因组DNA,采用16S rDNA基因扩增和Illumina测序技术将细菌归类;1块进行苏木精-伊红染色和RE病理分级。比较各组食管黏膜菌群构成差别,比较各组食管黏膜病理学改变。结果 每组6只完成研究,假手术组、蒸馏水组和香连片组食管下段黏膜属水平菌群构成:消化链球菌属丰度值分别为(2.5±0.5)%、(1.7±0.7)%和(2.5±0.3)%(P=0.040),螺杆菌属丰度值分别为(1.5±0.1)%、(0.9±0.4)%和(1.6±0.1)%(P=0.003);乳球菌属丰度值分别为(0.8±0.1)%、(0.4±0.2)%和(0.7±0.1)%(P=0.018)。蒸馏水组RE病理分级轻度1例、中度3例、重度2例;香连片组轻度6例,无中度及重度;2组间比较差异有统计学意义(P=0.015)。结论 RE大鼠灌服香连片后食管下段黏膜菌群构成发生改变,有益菌比例增加,从而起到治疗作用,这可能为RE的治疗提供新的方向。
【Abstract】Objective To investigate the effect of intragastric administration of Xianglian tablets on esophageal mucosal microbiota in rats with reflux esophagitis(RE). Methods Twenty-four healthy male SD rats were randomly divided into sham operation group, distilled water group(RE control group) and Xianglian tablets group(RE observation group), with 8 rats in each group. RE rat model was prepared by partial pyloric ligation and complete anterior gastric ligation. One week after operation, the RE observation group was treated with intragastric administration of Xianglian tablets dissolved in distilled water, while the sham operation group and RE control group were treated with intragastric administration of distilled water. Two weeks after medication, 2 samples of lower esophageal mucosa were separated in each rat for bacterial genomic DNA extraction and pathological observation. Esophageal mucosal genomic DNA was categorized using 16S rDNA gene amplification and Illumina sequencing techniques. Pathological changes of RE was observed by hematoxylin-eosin staining. Results Six rats survived in each group. Abundance values of lower esophageal mucosal microbiota at the genus level showed significant differences among the sham operation, RE control group and RE observation group: Peptostreptococcus (2.5±0.5)%,(1.7±0.7)% vs (2.5±0.3)%(P=0.040); Helicobacter (1.5±0.1)%,(0.9±0.4)% vs (1.6±0.1)%(P=0.003); Lactococcus (0.8±0.1)%,(0.4±0.2)% vs (0.7±0.1)%(P=0.018). Pathological grading results of mild, moderate, severe were 1, 3, 2 cases respectively in the RE control group. All 6 cases in the RE observation group were mild and there was a statistical difference in pathological grading between the two RE groups(P=0.015). Conclusion Esophageal mucosal microbiota changes in RE rats after treating with Xianglian tablets; the proportion of beneficial bacteria increases and it promotes the therapeutic effect on RE, which may provide a new direction for the treatment.
