自主专利桡动脉压迫器在经桡动脉冠状动脉介入术后应用的效果观察

• 摘要：

• 目的 观察自主专利桡动脉压迫器联合凝胶蓄冷冰袋在经桡动脉冠状动脉介入术后减轻患者肿胀、疼痛及提高舒适度的效果。方法 选取2017年6—10月在首都医科大学附属北京安贞医院行经桡动脉冠状动脉介入术的患者240例，完全随机分为对照组和观察组，每组120例。对照组术后采用传统清水冰袋持续冰敷12 h，观察组术后采用自制桡动脉压迫器联合凝胶蓄冷冰袋冰敷12 h。比较2组患者术后6、12 h肿胀程度和疼痛评分，以及术后12 h时Kolcaba舒适状况评分。结果 观察组术后6、12 h肿胀程度均明显低于对照组［（17.0±2.2）%比（23.2±2.1）%、（16.5±2.3）%比（21.5±1.8）%］，术后6、12 h疼痛视觉模拟量表评分均明显低于对照组［（5.6±1.8）分比（8.2±3.2）分、（3.2±2.1）分比（6.2±1.1）分］，术后12 h时Kolcaba舒适状况评分明显高于对照组［（92.3±3.2）分比（84.0±2.3）分］，差异均有统计学意义(均P＜0.001)。结论自制桡动脉压迫器联合凝胶蓄冷冰袋的使用能有效减少经桡动脉冠状动脉介入术后前臂肿胀和局部疼痛，提高患者舒适度。

• Objective To observe the effect of self-made radial artery compressor device combined with gel ice pack on postoperative swelling, pain and patient comfort after trans-radial percutaneous coronary artery intervention. Methods From June to October 2017, 240 patients who underwent coronary artery intervention via radial artery in Beijing Anzhen Hospital, Capital Medical University were randomly divided into control group and observation group, with 120 cases in each group. The control group was treated with traditional compress method with ice pack and the observation group was treated with self-made radial artery compressor combined with ice pack compress for 12 h after operation. Swelling degree and pain score were analyzed at 6 h and 12 h after operation. Kolcaba comfort score was analyzed at 12 h after operation. Results Swelling degrees at 6 h and 12 h after operation in the observation group were significantly less than those in the control group［（17.0±2.2）% vs （23.2±2.1）%, （16.5±2.3）% vs （21.5±1.8）%,P＜0.001］. Pain Visual Analogue Scale scores at 6 h and 12 h after operation in the observation group were significantly lower than those in the control group［（5.6±1.8） vs （8.2±3.2）, （3.2±2.1） vs （6.2±1.1）］(P＜0.001). At 12 h after operation, Kolcaba comfort score in the observation group was significantly higher than that in the control group［（92.3±3.2） vs （84.0±2.3）,P＜0.001］. ConclusionSelf-made radial artery compressor combined with gel ice pack can effectively reduce forearm swelling, local pain and improve patients′comfort after trans-radial coronary artery intervention.
