目的 探讨2型糖尿病患者血清微小RNA-31(miR-31)表达水平与并发糖尿病肾病(DN)的相关性,以及miR-31与机体炎症水平的相关性。方法 纳入2016年6月至2018年6月河北省沧州市人民医院的 2型糖尿病患者75例,其中无任何糖尿病相关血管并发症者25例(无并发症组),合并DN者25例(DN组),仅合并视网膜病变者25例(视网膜病变组);另选取同期健康志愿者50例(对照组)。应用实时聚合酶链反应技术检测各组受试者血清miR-31表达水平,流式细胞仪检测血清肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)和细胞间黏附分子1(ICAM-1)水平。结果 对照组和2型糖尿病患者的血清miR-31表达量比较,差异无统计学意义(P=0.528)。DN组的血清miR-31表达量明显低于无并发症组[(0.39±0.21)比(1.05±0.67)](P=0.001),而视网膜病变组的血清miR-31表达量与无并发症组比较差异无统计学意义(P=0.356)。DN组的血清ICAM-1水平明显高于无并发症组[(158.60±25.93)μg/L比(96.75±19.54)μg/L](P=0.001)。Pearson相关性分析结果 显示,DN组患者血清miR-31水平与TNF-α(r=-0.614,P=0.001)、IL-6(r=-0.452,P=0.026)、ICAM-1水平(r=-0.540,P=0.006)之间存在负相关。结论血清miR-31可作为2型糖尿病患者发生DN的生物学标志物,miR-31的表达与炎症因子TNF-α、IL-6、ICAM-1呈负相关。
Objective To investigate the relation among serum microRNA-31(miR-31) expression, diabetic nephropathy(DN) and inflammation in patients with type 2 diabetes mellitus. Methods From June 2016 to June 2018, 75 patients with type 2 diabetes mellitus in Cangzhou People′s Hospital, Hebei Province were enrolled, including 25 cases without diabetic complication(non-complication group), 25 cases with DN(DN group) and 25 cases with diabetic retinopathy(DR group); 50 healthy people were enrolled as control group. Serum miR-31 expression was detected by real-time polymerase chain reaction. Levels of serum tumor necrosis factor-α(TNF-α), interleukin-6(IL-6) and intercellular adhesion molecule-1(ICAM-1) were detected by flow cytometry. Results There was no significant difference in serum miR-31 expression between the control group and diabetic patients(P=0.528). Serum miR-31 expression in the DN group was significantly lower than that in the non-complication group[(0.39±0.21) vs (1.05±0.67)](P=0.001). There was no significant difference in serum miR-31 expression between the DR group and control group(P=0.356). Serum ICAM-1 level in the DN group was significantly higher than that in the non-complication group[(158.60±25.93)μg/L vs (96.75±19.54)μg/L](P=0.001). Pearson analysis showed that serum miR-31 expression in the DN group was negatively correlated with TNF-α(r=-0.614, P=0.001), IL-6(r=-0.452, P=0.026) and ICAM-1(r=-0.540, P=0.006). Conclusions Serum miR-31 can be used as a biomarker of DN in patients with type 2 diabetes mellitus. Expression of miR-31 is negatively correlated with inflammatory molecules TNF-α, IL-6 and ICAM-1.
