目的 探讨癫痫患者血浆中微小RNA(miR)-7、miR-146a的表达特点,分析其与脑电图异常的相关性。方法 选择2017年1月至2020年10月广州医科大学附属脑科医院神经科收治的105例癫痫患者(癫痫组),根据全数字化视频脑电图仪检测结果将其分为轻度异常组(36例)、中度异常组(38例)、重度异常组(31例),另选择同期63例健康志愿者为对照组。实时荧光聚合酶链反应法检测血浆miR-7、miR-146a表达水平,酶联免疫吸附试验法检测血浆白细胞介素1β(IL-1β)、IL-8、肿瘤坏死因子α(TNF-α)、IL-2水平。比较各组以上指标差异,分析miR-7、miR-146a表达与癫痫患者脑电图异常的相关性。结果 癫痫组血浆miR-7、miR-146a表达水平低于对照组[(0.45±0.12)比(1.06±0.31)、(0.56±0.21)比(1.15±0.39)],血浆IL-1β、IL-8、TNF-α、IL-2水平高于对照组,差异均有统计学意义(均P<0.001)。癫痫患者中脑电图重度异常组血浆miR-7、miR-146a表达低于中度异常组和轻度异常组,中度异常组低于轻度异常组;重度异常组血浆IL-1β、IL-8、TNF-α、IL-2水平高于中度异常组和轻度异常组,中度异常组高于轻度异常组,差异均有统计学意义(均P<0.05)。Pearson相关或Spearman秩相关分析结果显示血浆miR-7、miR-146a表达与脑电图异常程度以及血浆IL-1β、IL-8、TNF-α、IL-2水平均呈负相关,差异均有统计学意义(均P<0.05)。结论 癫痫患者血浆miR-7、miR-146a表达降低,miR-7、miR-146a表达缺失与癫痫患者脑电图严重程度有关。
Objective To investigate the expression characteristics of microRNA(miR)-7 and miR-146a in plasma of patients with epilepsy, and to analyze their correlation with abnormal electroencephalogram(EEG). Methods From January 2017 to October 2020, 105 patients with epilepsy(epilepsy group) admitted to Department of Neurology, the Affiliated Brain Hospital of Guangzhou Medical University were selected. They were divided into mildly abnormal group(36 cases), moderately abnormal group(38 cases) and severely abnormal group(31 cases) according to the detection results of full-digital video electroencephalograph. Another 63 healthy volunteers at the same time were selected as the control group. The expressions of plasma miR-7 and miR-146a were detected by real-time fluorescent polymerase chain reaction, and the levels of plasma interleukin 1β(IL-1β), IL-8, tumor necrosis factor-α (TNF-α) and IL-2 were detected by enzyme linked immunosorbent assay. The above indexes were compared among the groups. The correlation between the expressions of miR-7, miR-146a and the abnormal EEG in patients with epilepsy was analyzed. Results The expressions of plasma miR-7 and miR-146a in epilepsy group were lower than those in control group[(0.45±0.12) vs (1.06±0.31), (0.56±0.21) vs (1.15±0.39)], and the levels of plasma IL-1β, IL-8, TNF-α and IL-2 in epilepsy group were higher than those in control group(all P<0.001). The expressions of plasma miR-7 and miR-146a in severely abnormal group were lower than those in moderately abnormal group and mildly abnormal group, and those in moderately abnormal group were lower than those in mildly abnormal group; the levels of plasma IL-1β, IL-8, TNF-α and IL-2 in severely abnormal group were higher than those in moderately abnormal group and mildly abnormal group, and those in moderately abnormal group were higher than those in mildly abnormal group(all P<0.05). Pearson correlation or Spearman rank correlation analysis showed that the expressions of plasma miR-7 and miR-146a were negatively correlated with the abnormal degree of EEG and the levels of plasma IL-1β, IL-8, TNF-α and IL-2, and the differences were statistically significant(all P<0.05). Conclusion The expressions of plasma miR-7 and miR-146a decrease in patients with epilepsy, and the loss of expressions of miR-7 and miR-146a is related to the severity of epileptic EEG.
