单次输注右美托咪定对脊髓栓系患者全身麻醉术中尿量及电解质水平的影响

• 目的    观察单次输注右美托咪定对脊髓栓系患者全身麻醉术中尿量及电解质水平的影响。方法    选取2016年9—12月于全身麻醉下行脊髓栓系松解术的患者60例，完全随机分为A、B组，各30例。A组麻醉诱导前给予右美托咪定0.8 μg/kg静脉泵注，B组给予0.9%氯化钠注射液静脉泵注，均10 min泵完。术中记录2组患者给予麻醉诱导前药物后1、2、3 h尿量及血浆电解质情况。结果    给予麻醉诱导前药物后1 h，A组与B组尿量差异无统计学意义［（131±52）ml比（137±63）ml］（P>0.05）；给药后2、3 h，A组尿量明显多于B组［（527±136）ml比（231±44）ml、（476±162）ml比（329±85）ml］及本组给药后1 h，差异均有统计学意义（均P<0.05）。给予麻醉诱导前药物后1、2、3 h，2组患者K+、Na+、Ca2+浓度均有所波动，但都在正常范围内。结论    单次输注右美托咪0.8 μg/kg可增加脊髓栓系患者全身麻醉术中尿量，但不影响患者体内的电解质水平。

• Effect of single dose of dexmedetomidine on urine volume and electrolytes during general anesthesia in tethered spinal cord patients

  【Abstract】Objective    To analyze the effect of single dose of dexmedetomidine on urine volume and electrolytes during general anesthesia in tethered spinal cord patients. Methods    Sixty patients undergoing tethered spinal cord lysis under general anesthesia from September to December 2016 in Tinghua University Yuquan Hospital were randomized into group A and group B, with 30 cases in each group. Group A had intravenous infusion of dexmedetomidine 0.8 μg/kg and group B had intravenous infusion of 0.9% sodium chloride before anesthesia induction. Urine volume and plasma electrolytes were measured 1, 2, 3 h after drug administration during operation. Results    There was no significant difference of 1 h urine volume between group A and group B［（131±52）ml vs （137±63）ml］（P＞0.05）. Urine volumes at 2, 3 h after drug administration in group A were significantly more than those in group B［（527±136）ml vs （231±44）ml, （476±162）ml vs （329±85）ml］（P＜0.05）. Plasma concentrations of K＋, Na＋, Ca2＋ fluctuated during operation in both groups but they were still within the normal range. Conclusions    ingle infusion of dexmedetomidine 0.8 μg/kg can increase intraoperative urine volume in tethered spinal cord patients but it can not affect electrolyte levels.
