经颅磁刺激结合阿戈美拉汀治疗难治性抑郁症的临床效果

  • 目的    分析经颅磁刺激结合阿戈美拉汀治疗难治性抑郁症的临床效果。方法    选取2016年1月至2017年1月解放军一八一医院收治的难治性抑郁症患者102例作为研究对象。采用随机数字表法分为观察组和对照组,各51例。对照组给予阿戈美拉汀50 mg/次,1次/d睡前服用,观察组在对照组基础上给予经颅磁刺激治疗。对比2组临床效果及不良反应发生情况。结果    2组入院时汉密尔顿抑郁量表(HAMD)评分、临床疗效总评量表病情严重程度(CGI-SI)评分、威斯康星卡片分类测验(WCST)评分差异无统计学意义(P>0.05)。治疗3周后2组HAMD评分、CGI-SI评分均较入院时降低,WCST评分较入院时升高,且观察组评分改善情况优于对照组[(13.7±5.0)分比(20.2±3.0)分、(2.4±0.7)分比(3.5±0.9)分、(82±7)分比(53±6)分],差异均有统计学意义(P<0.05)。观察组总有效率高于对照组[88.2%(45/51)比68.6%(35/51)],差异有统计学意义(P<0.01)。2组均未出现严重不良反应。结论    经颅磁刺激结合阿戈美拉汀治疗难治性抑郁症的临床效果及安全性良好。

  • Clinical effect of transcranial magnetic stimulation combined with agomelatine on refractory depression

    【Abstract】Objective    To analyze the clinical effect of transcranial magnetic stimulation combined with agomelatine on refractory depression. Methods    Totally 102 patients with refractory depression were enrolled from January 2016 to January 2017 in the 181st Hospital of Chinese People′s Liberation Army, and they were randomly divided into observation group and control group, with 51 cases in each group. Patients in control group took agomelatine 50 mg/time, 1 time/d before bedtime; patients in observation group were treated with transcranial magnetic stimulation combined with agomelatine. Clinical efficacy and adverse reactions were analyzed. Results    On admission, scores of the Hamilton Depression Rating Scale(HAMD), the Clinical Global Impression-Severity of Illness(CGI-SI) and the Wisconsin Card Sorting Test(WCST) had no significant differences between groups(P>0.05). After 3 weeks of treatment, the HAMD score and the CGI-SI score were significantly lower and the WCST score was significantly higher than those before treatment, and there were significant differences between observation group and control group[(13.7±5.0)points vs (20.2±3.0)points, (2.4±0.7)points vs (3.5±0.9)points, (82±7)points vs (53±6)points](P<0.05). The total effective rate of observation group was significantly higher than that of control group[88.2% (45/51) vs 68.6% (35/51)](P<0.01). No serious adverse events occurred in both groups. Conclusion    Transcranial magnetic stimulation combined with agomelatine has good clinical efficacy and safety in treatment of refractory depression.
