西帕依固龈液联合牙周治疗对妊娠期牙周炎的效果观察

• 目的    观察西帕依固龈液联合牙周治疗对妊娠期牙周炎的临床效果。方法    选取2013年2月至2017年6月北京医院国家老年医学中心口腔科就诊的妊娠期牙周炎患者160例，完全随机分为2组，各80例,对照组给予牙周基础治疗，观察组给予西帕依固龈液含漱配合牙周基础治疗，含漱持续到牙周治疗完成后4周。分别于治疗前和治疗完成6周后检查2组患者菌斑指数、龈沟出血指数、探诊深度和龈沟液中肿瘤坏死因子α和白细胞介素6含量,记录药物不良反应及不良妊娠结局。结果    治疗前，2组患者牙周相关指标和龈沟液中炎症因子含量差异均无统计学意义（均P>0.05）;治疗后，2组患者菌斑指数、龈沟出血指数和龈沟液中肿瘤坏死因子α和白细胞介素6含量均低于治疗前［对照组：(2.8±0.3)分比（3.7±0.3）分，(2.66±0.41)分比（3.39±0.37）分，(6.1±1.0)ng/L比（7.4±1.3）ng/L，(2.53±0.24)ng/L比（3.47±0.29）ng/L;观察组：(2.1±0.4)分比（3.6±0.6）分，(2.03±0.52)分比（3.37±0.19）分，(4.3±0.8)ng/L比（7.6±1.4）ng/L，(1.61±0.20)ng/L比（2.99±0.45）ng/L］，且观察组菌斑指数、龈沟出血指数和龈沟液中肿瘤坏死因子α和白细胞介素6含量均低于对照组，差异均有统计学意义（均P<0.05）。治疗期间2组患者均未发现药物不良反应，早产低体质量儿发生率2组间比较差异无统计学意义（P>0.05）。结论    牙周基础治疗联合西帕依固龈液含漱，可更好地改善妊娠期牙周炎患者牙周状况，减轻局部炎症，且用药安全。

• Objective    To observe the effect of Xipayi mouth rinse combined with periodontal therapy on pregnancy periodontitis. Methods    Totally 160 pregnant patients with periodontitis in Beijing Hospital from February 2013 to June 2017 were randomly divided into control group and observation group, with 80 cases in each group. The control group had periodental non-surgical therapy; the observation group was treated by Xipayi mouth rinse combined with periodental therapy. Plaque index（PI）, sulcular bleeding index(SBI), probing depth（PD）, tumor necrosis factor-α（TNF-α） and interleukin-6（IL-6） in gingival crevicular fluid were detected before and 6 months after treatment. Drug adverse reactions and adverse pregnancy outcomes were observed. Results   There were no significant differences of periodental indexes and levels of inflammatory factors in gingival crevicular fluid between groups before treatment. After treatment, PLI, SBI, PD, TNF-α and IL-6 levels in gingival crevicular fluid were significantly lower than those before treatment in both groups［control group： (2.8±0.3)points vs （3.7±0.3）points， (2.66±0.41)points vs （3.39±0.37）points， (6.1±1.0)ng/L vs （7.4±1.3）ng/L， (2.53±0.24)ng/L vs （3.47±0.29）ng/L; observation group： (2.1±0.4)points vs （3.6±0.6）points， (2.03±0.52)points vs （3.37±0.19）points， (4.3±0.8)ng/L vs （7.6±1.4）ng/L， (1.61±0.20)ng/L vs （2.99±0.45）ng/L］; PLI, SBI, PD, TNF-α and IL-6 levels in gingival crevicular fluid in observation group were significantly lower than those in control group（P<0.05）. No adverse drug reactions occurred in both groups. Low body weight premature infant rate showed no significant difference between groups（P>0.05）. Conclusion    Xipayi mouth rinse combined with periodontal therapy is safe for pregnant women with periodontitis and it can effectively improve periodontal status and alleviate local inflammation.
