【摘要】目的 探讨血清瘦素及脂联素水平与三阴性乳腺癌(TNBC)的关系。方法 选取2017年6月至2018年1月在新疆医科大学附属肿瘤医院乳腺外科诊断为乳腺癌并行手术治疗的230例患者作为观察组(TNBC 65例、非TNBC 165例),选取同期在本院体检中心进行体检的60名健康妇女作为对照组。比较2组血清瘦素及脂联素水平,分析血清瘦素及脂联素水平与临床指标的关系。结果 观察组血清瘦素水平明显高于对照组、血清脂联素水平明显低于对照组[(1.4±0.5)μg/L比(1.0±0.5)μg/L、(8.3±2.7)mg/L比(10.6±2.6)mg/L],差异均有统计学意义(均P<0.05)。观察组血清脂联素、瘦素水平与患者是否肥胖、肿瘤TNM分期、淋巴结有无转移有相关性(P<0.05),而与年龄、民族、肿瘤分子分型是否三阴性及肿瘤组织学分级无明显相关性(P>0.05)。多变量Logistic回归分析结果显示血清瘦素水平与乳腺癌的发生呈正相关(比值比=1.131,95%置信区间:1.016~1.208,P<0.05),血清脂联素水平与乳腺癌的发生呈负相关(比值比=0.876,95%置信区间:0.476~0.982,P<0.05)。结论 血清瘦素水平增高和脂联素水平降低是乳腺癌发生的危险因素,与肿瘤分子分型是否三阴性无明显关系。
【Abstract】Objective To analyze the relation among serum leptin, adiponectin and triple-negative breast cancer(TNBC). Methods From June 2017 to January 2018, 230 breast cancer patients(observation group, 65 TNBC, 165 non-TNBC) and 60 healthy women(control group) were enrolled in Xinjiang Tumor Hospital Affiliated to Xinjiang Medical University. Serum levels of leptin and adiponectin were detected. The relation among leptin, adiponectin and related clinical indicators was analyzed. Results Serum leptin level in observation group was significantly higher and serum adiponectin level was significantly lower than those in control group[(1.4±0.5)μg/L vs (1.0±0.5)μg/L, (8.3±2.7)mg/L vs (10.6±2.6)mg/L](P<0.05). In observation group, leptin and adiponectin were related to obesity, TNM stage and lymph node metastasis(P<0.05). There were no significant relations among leptin, adiponectin levels and age, nationality, tumor molecular type(whether TNBC or not), histological grade(P>0.05). Multivariate Logistic regression showed that serum leptin level was positively correlated with breast cancer(odds ratio=1.131, 95% confidence interval: 1.016-1.208, P<0.05); serum adiponectin level was negatively correlated with breast cancer(odds ratio=0.876, 95% confidence interval: 0.476-0.982, P<0.05). Conclusions High leptin and low adiponectin are risk factors of breast cancer. There is no relation among leptin, adiponectin and TNBC.
