【摘要】目的 探讨老年缺血性脑血管病患者血管内皮细胞功能指标与沉默信息调节因子1(SIRT1)的相关性。方法 选取2016年9月至2018年2月海南省人民医院收治的老年缺血性脑血管病患者160例,其中短暂性脑缺血发作80例,脑梗死80例;另选择我院同期体检健康的志愿者80名为对照组,比较3组研究对象血浆SIRT1和血管内皮细胞功能指标[氧化型低密度脂蛋白胆固醇(ox-LDL)、细胞间黏附分子1(ICAM-1)、内皮素1、血管细胞黏附分子1(VCAM-1)、血小板衍生因子(PDGF)、肿瘤坏死因子α(TNF-α)、一氧化氮、白细胞介素6(IL-6)]水平,分析血浆SIRT1与血管内皮细胞功能指标的相关性。结果 脑梗死组和短暂性脑缺血发作组血浆SIRT1水平均高于对照组[(19.2±4.4)、(15.2±3.7)μmol/L比(9.3±1.3)μmol/L],且脑梗死组高于短暂性脑缺血发作组,差异均有统计学意义(均P<0.05)。脑梗死组和短暂性脑缺血发作组ox-LDL、ICAM-1、内皮素1、VCAM-1、PDGF、TNF-α、IL-6水平均高于对照组,且脑梗死组高于短暂性脑缺血发作组;而一氧化氮水平均低于对照组,且脑梗死组低于短暂性脑缺血发作组,差异均有统计学意义(均P<0.05)。相关性分析结果显示,ox-LDL、ICAM-1、内皮素1、VCAM-1、PDGF、TNF-α、IL-6水平与SIRT1呈正相关(r=0.625、0.658、0.602、0.563、0.623、0.616、0.688,均P<0.05),一氧化氮水平与SIRT1呈负相关(r=-0.697,P<0.05)。结论 SIRT1在脑梗死患者血浆中呈高表达,其与内皮细胞功能的恶化程度呈正相关,SIRT1与血管内皮细胞功能指标密切相关,可为老年缺血性脑血管病患者预后判断提供依据。
【Abstract】Objective To investigate the relation between vascular endothelial function indexes and silencing information regulator 1(SIRT1) in senile patients with ischemic cerebrovascular disease. Methods A total of 160 senile patients with ischemic cerebrovascular disease[80 patients with transient ischemic attack(TIA) and 80 patients with cerebral infarction(CI)] were enrolled from September 2016 to February 2018 in Hainan General Hospital. Eighty healthy volunteers were enrolled as control group. Levels of plasma SIRT1 and vascular endothelial function indexes including oxidized low density lipoprotein cholesterol(ox-LDL), intercellular adhesion molecule-1(ICAM-1), endothelin-1(ET-1), vascular cell adhesion molecule-1(VCAM-1), platelet derived growth factor(PDGF), tumor necrosis factor-α(TNF-α), nitric oxide(NO) and interleukin-6(IL-6) were tested. Relation between SIRT1 level and these vascular endothelial function indexes was analyzed. Results There were significant differences of plasma SIRT1 level among CI group, TIA group and control group[(19.2±4.4), (15.2±3.7)μmol/L vs (9.3±1.3)μmol/L](all P<0.05). There were significant differences of ox-LDL, ICAM-1, ET-1, VCAM-1, PDGF, TNF-α and IL-6 levels among the 3 groups(all P<0.05). Ox-LDL, ICAM-1, ET-1, VCAM-1, PDGF, TNF-α, IL-6 and NO were positively correlated with SIRT1(r=0.625, 0.658, 0.602, 0.563, 0.623, 0.616, 0.688; all P<0.05); NO was negatively correlated with SIRT1(r=-0.697, P<0.05). Conclusions CI patients show high expression of SIRT1, which is positively correlated with vascular endothelial function deterioration. SIRT1 may be an indicator of vascular endothelial function and the prognosis of ischemic cerebrovascular disease in the elderly.
