【摘要】目的 探究不同体位光疗对高胆红素血症早产儿的干预效果。方法 选取江苏省徐州市儿童医院2016年2月至2017年2月收治的采用传统仰卧位光疗的高胆红素血症早产儿42例作为对照组,另选取本院2017年3月至2018年3月收治的采用俯卧位光疗的高胆红素血症早产儿42例作为观察组。对比2组临床疗效、生理指标变化及不良反应发生率。结果 观察组临床总有效率明显高于对照组[97.6%(41/42)比81.0%(34/42)](P=0.034)。观察组光疗1、2、3、4、5、6 h血氧饱和度均明显高于对照组[(97.7±1.5)%比(96.0±1.7)%、(97.9±1.5)%比(96.2±1.7)%、(98.1±1.4)%比(96.6±1.4)%、(98.3±1.3)%比(96.3±1.4)%、(98.4±1.2)%比(96.4±1.2)%、(98.5±1.9)%比(96.4±1.2)%](均P<0.01);2组间心率比较,差异无统计学意义(P>0.05)。观察组不良反应发生率明显低于对照组[9.5%(4/42)比38.1%(16/42)](χ2=9.450, P=0.002)。结论 俯卧位光疗用于高胆红素血症早产儿治疗可有效提高临床疗效,改善血氧饱和度,降低不良反应发生率。
【Abstract】Objective To investigate the effects of phototherapy with different body positions on premature infants with hyperbilirubinemia. Methods Forty-two preterm infants with hyperbilirubinemia undergoing conventional phototherapy with supine position from February 2016 to February 2017 in Xuzhou Children′s Hospital were enrolled as a control group and 42 preterm infants undergoing phototherapy with prone position from March 2017 to March 2018 were observation group. Clinical efficacy, physiological indicators and adverse reactions were analyzed. Results Total clinical effective rate in the observation group was significantly higher than that in the control group[97.6%(41/42) vs 81.0%(34/42)](P=0.034). Blood oxygen saturation in the observation group was significantly higher than that in the control group 1, 2, 3, 4, 5, 6 h after phototherapy[(97.7±1.5)% vs (96.0±1.7)%, (97.9±1.5)% vs (96.2±1.7)%, (98.1±1.4)% vs (96.6±1.4)%,(98.3±1.3)% vs (96.3±1.4)%, (98.4±1.2)% vs (96.4±1.2)%, (98.5±1.9)% vs (96.4±1.2)%](all P<0.01). Heart rate showed no significant difference between groups(all P>0.05). Incidence of adverse reactions in the observation group was significantly lower than that in the control group[9.5%(4/42) vs 38.1%(16/42)](χ2=9.450, P=0.002). Conclusion Phototherapy with prone position is effective in the treatment of hyperbilirubinemia in preterm infants; it can improve blood oxygen saturation and reduce adverse reactions.
