老年恶性颌下腺肿瘤预后的影响因素分析

• 【摘要】目的    探讨老年恶性颌下腺肿瘤预后的影响因素。方法    选择2013年1月至2018年6月湖北省荆州市第二人民医院收治的老年恶性颌下腺肿瘤患者137例作为研究对象，收集相关临床资料，随访36个月预后情况，根据是否生存和死亡评价预后。采用单因素分析老年恶性颌下腺肿瘤预后的相关影响因素；采用多因素Logistic回归分析老年恶性颌下腺肿瘤预后的危险因素。结果    所有研究对象完成随访36个月，生存78例（56.9%），死亡59例（43.1%）。单因素分析表明，死亡组临床分期Ⅳ期者多于生存组，低分化多于生存组，复发多于生存组，淋巴结转移多于生存组，差异均有统计学意义（均P＜0.05）。多因素Logistic回归分析显示，临床分期Ⅳ期（比值比＝2.489，95%置信区间：1.476～3.254，P＜0.001）、低分化（比值比＝1.967，95%置信区间：1.286～2.870，P=0.005）、复发（比值比＝1.628，95%置信区间：1.176～2.453，P=0.032）和淋巴结转移（比值比＝2.971，95%置信区间：1.687～4.108，P＜0.001）为影响老年恶性颌下腺肿瘤预后的危险因素。结论    临床分期、分化程度、复发和淋巴结转移为影响年恶性颌下腺肿瘤预后的危险因素。

• 【Abstract】Objective    To investigate the influence factors of prognosis in elderly patients with malignant submandibular gland tumor. Methods    Totally 137 elderly patients with malignant submandibular gland tumor admitted to the Second Hospital of Jingzhou from January 2013 to June 2018 were enrolled. They were followed up for 36 months and divided into survival group and death group. Prognostic risk factors were analyzed by univariate and multivariate logistic regression. Results    All subjects completed follow-up; 78 cases(56.9%) survived and 59 cases(43.1%) died. Univariate analysis showed that ratios of clinical stage Ⅳ, low differentiation, recurrence and lymphatic metastasis in the death group were significantly higher than those in the survival group（all P＜0.05）. Multivariate logistic regression analysis showed that clinical stage Ⅳ［odds ratio（OR）=2.489, 95% confidence interval（CI）: 1.476-3.254, P＜0.001］, low differentiation(OR=1.967, 95% CI: 1.286-2.870, P=0.005), recurrence(OR=1.628, 95% CI: 1.176-2.453, P=0.032) and lymphatic metastasis(OR=2.971, 95% CI: 1.687-2.108, P＜0.001) were risk factors of the prognosis. Conclusion    Clinical stage, differentiation degree, recurrence and lymphatic metastasis are risk factors of the prognosis in elderly patients with malignant submandibular gland tumor.
