磷霉素氨丁三醇散在膀胱灌注相关泌尿系感染患者中治疗效果分析

• 【摘要】目的    探讨磷霉素氨丁三醇散在膀胱灌注相关泌尿系感染患者中的治疗效果。方法    选取2016年4—12月在北京大学第一医院泌尿外科门诊进行膀胱灌注期间出现泌尿系感染的159例患者，根据药物干预的不同分成A、B、C组。A组（102例）患者口服磷霉素氨丁三醇散治疗，B组（33例）患者口服头孢克洛治疗，C组（24例）患者口服左氧氟沙星治疗。3组患者均治疗7 d。比较3组患者的治疗总有效率、不良反应发生率和患者满意率。结果    A组治疗总有效率及不良反应发生率均明显高于B、C组［91.2%（93/102）比78.8%（26/33）、70.8%（17/24），37.3%（38/102）比0、8.3%（2/24）］，差异均有统计学意义（均P＜0.05）。3组患者满意率比较，差异无统计学意义（P=0.893）。结论    磷霉素氨丁三醇散在膀胱灌注相关泌尿系感染患者中治疗效果优于头孢克洛、左氧氟沙星，虽然可能发生腹泻等不良反应，但其停药后症状立即消失，患者满意率与使用头孢克洛和左氧氟沙星相当。

• 【Abstract】Objective    To explore the therapeutic effect of fosfomycin trometamol powders on urinary infection associated with bladder perfusion. Methods    Clinical data of 159 patients suffering from urinary infection during bladder perfusion in Peking University First Hospital from April to December 2016 were retrospectively analyzed. The patients were divided into three groups, the group A（102 cases） took fosfomycin trometamol powders; the group B（33 cases） took cefaclor; the group C（24 cases） took levofloxacin; all patients were treated for 7 days. Total effective rate, incidence of adverse reactions and satisfaction rate of patients were analyzed. Results    Total effective rate and incidence of adverse reactions in group A were significantly higher than those in group B and C［91.2%(93/102) vs 78.8%(26/33), 70.8%(17/24); 37.3%(38/102) vs 0, 8.3%(2/24)］(all P＜0.05). There was no significant difference in satisfaction rate of patients among groups(P=0.893). Conclusions    Fosfomycin trometamol powders show better clinical efficacy and equal satisfaction rate of patients compared to cefaclor and levofloxacin in treatment of urinary infection associated with bladder perfusion. Common adverse reactions such as diarrhea are tolerable and disappear immediately after withdrawal.
