不同健康状况人工关节置换术患者的血液指标及术后下肢深静脉血栓形成情况比较

• 【摘要】目的    比较不同健康状况人工关节置换术患者的血液指标及术后下肢深静脉血栓形成情况。方法    选取2015年12月至2017年12月于新疆医科大学第六附属医院治疗的人工关节置换患者100例作为研究对象。根据患者不同健康情况分为健康组与疾病组，各50例。比较2组患者的凝血相关指标，血糖、血脂指标及下肢深静脉血栓形成情况。结果    疾病组的凝血酶原时间、活化部分凝血活酶时间明显短于健康组［（12.3±2.1）s比（13.3±2.3）s、（39±3）s比（45±4）s］，差异均有统计学意义（均P＜0.05）。2组血小板计数水平差异无统计学意义（P>0.05）。疾病组空腹血糖、总胆固醇、三酰甘油高于健康组，高密度脂蛋白胆固醇低于健康组［（6.8±1.2）mmol/L比（4.5±1.3）mmol/L、（5.7±0.7）mmol/L比（4.0±0.6）mmol/L、（2.8±0.3）mmol/L比（1.3±0.4）mmol/L、（1.30±0.20）mmol/L比（1.40±0.20）mmol/L］，差异均有统计学意义（均P＜0.05）。疾病组术后下肢深静脉血栓发生率高于健康组［60.0%（30/50）比30.0%（15/50）］，差异有统计学意义（P＜0.05）。结论    人工关节置换手术前合并高血压、糖尿病、高脂血症的患者血液处于高凝状态、血脂指标差，下肢深静脉血栓发生率高。

• 【Abstract】Objective    To analyze the levels of blood indicators and deep venous thrombosis of lower extremities in patients with different health condition undergoing artificial joint replacement. Methods    A total of 100 patients undergoing artificial joint replacement in the Sixth Affiliated Hospital of Xinjiang Medical University from December 2015 to December 2017 were enrolled. According to the different health condition of patients, they were divided into health group and disease group, with 50 cases in each group. Coagulation indexes, blood glucose, lipids and deep venous thrombosis of lower extremity were analyzed. Results    Prothrombin time and activated partial thromboplastin time in the disease group were significantly shorter than those in the healthy group［（12.3±2.1）s vs （13.3±2.3）s, （39±3）s vs （45±4）s］(both P<0.05). There was no significant difference of platelet count between groups(P>0.05). Levels of fasting blood glucose, total cholesterol and triglyceride in the disease group were significantly higher and the level of high density lipoprotein cholesterol was significantly lower than those in the healthy group［（6.8±1.2）mmol/L vs （4.5±1.3）mmol/L, （5.7±0.7）mmol/L vs （4.0±0.6）mmol/L, （2.8±0.3）mmol/L vs （1.3±0.4）mmol/L, （1.30±0.20）mmol/L vs （1.40±0.20）mmol/L］(all P<0.05). Incidence of lower extremity deep venous thrombosis in the disease group was significantly higher than that in the healthy group［60.0%(30/50) vs 30.0%(15/50)］(P<0.05). Conclusion    Blood of patients with hypertension, diabetes and hyperlipidemia is in hypercoagulative state and the blood lipid indexes are poor, leading to a high incidence of lower extremity deep venous thrombosis following artificial joint replacement.
