瑞巴派特对慢性萎缩性胃炎患者胃蛋白酶和胃泌素及炎性因子水平的影响

• 【摘要】目的    探讨瑞巴派特对慢性萎缩性胃炎（CAG）患者胃蛋白酶、胃泌素及炎性因子水平的影响。方法    纳入2017年7月至2018年6月浙江省中西医结合医院收治的112例CAG患者，按照随机数字表法分为观察组（56例）与对照组（56例）。对照组采用四联疗法治疗，观察组在对照组基础上联合瑞巴派特治疗。以7 d为1个疗程，2组患者均治疗2个疗程。比较治疗前后2组患者幽门螺杆菌（Hp）感染情况和胃肠激素、胃蛋白酶原（PG）及炎性因子水平。结果    治疗后4周，观察组Hp感染阳性率明显低于对照组［21.4%(12/56)比39.3%(22/56)］（P=0.04）。治疗后，2组胃泌素、PGⅠ、PGⅡ水平和PGⅠ/PGⅡ比值均高于治疗前，且观察组高于对照组，内皮素、肿瘤坏死因子α、白细胞介素2、白细胞介素6水平均低于治疗前，且观察组低于对照组［（61±9）ng/L比（71±10）ng/L、（4.7±1.0）ng/L比（5.4±1.6）ng/L、（313±24）μg/L比（366±25）μg/L、（18±10）ng/L比（22±6）ng/L］，差异均有统计学意义（均P＜0.05）。结论    瑞巴派特联合四联疗法对CAG患者进行治疗，能有效降低患者Hp感染率，保护胃黏膜屏障，改善胃内各腺体的分泌，并减轻机体炎性反应。

• 【Abstract】Objective    To investigate the effect of rebamipide on pepsin, gastrin and inflammatory factors in patients with chronic atrophic gastritis(CAG). Methods    From July 2017 to June 2018, 112 patients with CAG were enrolled in Zhejiang Integrated Traditional Chinese and Western Medicine Hospital. The patients were randomly divided into observation group(56 cases) and control group(56 cases). The control group was treated with routine quadruple therapy; the observation group was treated with rebapacit on the basis of routine therapy; 1 course of treatment was 7 days and both groups were treated for 2 courses. Helicobacter pylori(Hp) infection, serum levels of gastrointestinal hormones, pepsinogen(PG) and inflammatory factors were analyzed before and after treatment. Results    Four weeks after treatment, the positive rate of Hp infection in observation group was significantly lower than that in control group［21.4%(12/56) vs 39.3%(22/56)］(P=0.04). Levels of serum gastrin, PGⅠ, PGⅡ and PGⅠ/PGⅡ ratio significantly increased after treatment in both groups and the levels in observation group were higher than those in control group; levels of serum endothelin, tumor necrosis factor-α, interleukin-2 and interleukin-6 significantly decreased after treatment in both groups, and they were lower in observation group than those in control group［（61±9）ng/L vs （71±10）ng/L, （4.7±1.0）ng/L vs （5.4±1.6）ng/L, （313±24）μg/L vs （366±25）μg/L, （18±10）ng/L vs （22±6）ng/L］（all P＜0.05）. Conclusion    Rebamipide combined with quadruple therapy treating CAG can effectively reduce Hp infection rate, protect gastric mucosa, improve gastrointestinal hormone secretion and reduce inflammatory response.
