  • 目的 探讨经皮椎体后凸成形术(PKP)中应用人工骨修复材料(ABRM)对骨水泥凝固温度及术后疼痛的影响。方法 选取20169月至20199月北京市普仁医院微创脊柱骨科收治的骨质疏松性椎体压缩骨折并行PKP的患者112例。完全随机分为对照组和观察组,各56例。对照组病变椎体内单纯注射骨水泥,观察组病变椎体内注射骨水泥复合人工骨修复材料。比较2组术前及术后3 d、术后13个月腰背部疼痛视觉模拟量表(VAS)评分及术中第1022分钟的凝固物内部及表面温度、峰值温度。结果 所有患者手术顺利完成,均获得随访。观察组术后3 d腰背部疼痛VAS评分高于对照组[(5.0±1.0)分比(3.1±1.2)分],差异有统计学意义(P<0.05)。观察组凝固物内部及表面峰值温度均低于对照组[(72.4±5.6)℃比(87.2±7.6)℃、(52.1±4.3)℃比(64.2±5.1)℃],差异均有统计学意义(均P<0.05)。结论 PKP中应用ABRM可使骨水泥凝固温度明显降低,止痛效果在术后3 d较单纯应用骨水泥者下降。

  • Objective To investigate the effect of artificial bone repair material (ABRM) on bone cement setting temperature and postoperative pain in percutaneous kyphoplasty (PKP).Methods From September 2016 to September 2019, 112 patients with osteoporotic vertebral compression fracture and PKP admitted to Beijing Puren Hospital were selected. They were randomly divided into control group and observation group, with 56 cases in each group. The control group was injected with bone cement only, the observation group was injected with bone cement composite  and ABRM.Visual analogue scale (VAS) scores of lower back pain were compared between the two groups before and 3 days after surgery, 1 and 3 months after surgery, and the internal and surface temperatures and peak temperatures of the solidified substance in the 10-22 minutes during surgery were compared between the two groups.Results All patients had successfully completed the operation and were followed up. The VAS score of lower back pain on the 3rd postoperative day in the observation group was higher than that in the control group (5.0±1.0) vs (3.1±1.2) P<0.05. The internal and surface peak temperatures of the coagulum in the observation group were lower than those in the control group (72.4±5.6) vs (87.2±7.6), (52.1±4.3) vs (64.2±5.1)℃, All P<0.05.Conclusion The PKP application of ABRM can significantly reduce the setting temperature of bone cement, and the analgesic effect is decreased 3 days after the operation compared with applied bone cement alone.
