中老年高血压患者合并代谢综合征的危险因素分析

• 摘要：

• 目的 探讨中老年高血压患者合并代谢综合征（MS）的危险因素。方法 选取2018年8月至2019年5月中南大学湘雅医院心血管内科收治的244例中老年高血压患者作为研究对象，根据患者是否合并MS将其分为MS组（160例）和非MS组（84例），比较2组患者一般资料、体格检查情况、血脂及血糖指标水平，采用多因素Logistic回归模型分析影响中老年高血压患者合并MS的危险因素。结果 MS组男性、高血压病程≥5年者比例均高于非MS组［53.8%（86/160）比29.8%（25/84）、58.7%（94/160）比16.7%（14/84）,均P<0.05］。MS组患者腹围、体重指数、三酰甘油、血糖及糖化血红蛋白水平均高于非MS组，高密度脂蛋白胆固醇水平低于非MS组（均P<0.05）。Logistic回归分析结果 显示，男性（比值比=2.577，95%置信区间：1.365~4.865，P=0.004）、高血压病程≥5年（比值比=6.249，95%置信区间：3.118~12.525，P<0.001）是中老年高血压患者合并MS的独立危险因素。结论 男性及高血压病程≥5年是中老年高血压患者并发MS的主要危险因素。

• Objective To investigate the risk factors of metabolic syndrome（MS） in middle aged and elderly patients with hypertension. Methods A total of 244 middle aged and elderly patients with hypertension in Xiangya Hospital, Central South University from August 2018 to May 2019 were selected. According to whether the patients had MS, they were divided into MS group（160 cases）and non-MS group（84 cases）. The general data, physical examination, blood lipids and glucose levels were compared between both groups. Multivariate Logistic regression analysis was used to analyze the risk factors of middle aged and elderly patients with hypertension complicated with MS. Results The proportion of male and hypertension duration≥5 years in MS group were higher than those in non-MS group［53.8%（86/160） vs 29.8%（25/84）， 58.7%（94/160） vs 16.7%（14/84）, both P<0.05］. The abdominal circumference, body mass index, levels of triglyceride, blood glucose and glycosylated hemoglobin in MS group were higher than those in non-MS group(all P<0.05). Logistic regression analysis showed that male （odds ratio=2.577， 95% confidence interval： 1.365-4.865， P=0.004） and hypertension duration≥5 years （odds ratio=6.249， 95% confidence interval： 3.118-12.525， P<0.001） were the independent risk factors in middle aged and elderly hypertension patients complicated with MS. Conclusion Male and hypertension duration≥5 years are the main risk factors of MS in middle aged and elderly patients with hypertension.
