高原地区高血压病患者诊室血压和家庭自测血压及24 h动态血压的达标率一致性分析

• 摘要：

• 目的 研究分析高原地区高血压病患者诊室血压、家庭自测血压及24 h动态血压的血压达标率一致性。方法 选取2017年10月至2019年12月青海省心脑血管病专科医院收治的高原地区高血压病患者186例为研究对象，用诊室血压、家庭自测血压及24 h动态血压3种方法监测患者血压，比较不同血压监测方法的血压达标率，并分析血压达标率的一致性。结果 24 h动态血压监测的血压达标率明显高于诊室血压和家庭自测血压［35.5%（66/186）比20.4%（38/186）、16.1%（30/186）］，差异均有统计学意义（均P＜0.05）。诊室血压达标率与24 h动态血压达标率具有一致性（Kappa=0.151，P=0.001），家庭自测血压达标率与24 h动态血压达标率具有一致性（Kappa=0.194，P＜0.001），诊室血压达标率与家庭自测血压达标率不具有一致性（Kappa=0.043，P=0.283）。结论 不同血压监测方法监测高原地区高血压病患者血压，其中24 h动态血压达标率最高，与诊室血压和家庭自测血压达标率均具有一致性。

• Objective To investigate the consistency of attainment rate of clinic blood pressure, home blood pressure and 24 h ambulatory blood pressure among patients with hypertension in plateau area. Methods From October 2017 to December 2019， 186 patients with plateau area hypertension admitted to Qinghai Province Cardiovascular and Cerebrovascular Disease Hospital were enrolled. The clinic blood pressure (CBP), home blood pressure (HBP) and 24 h ambulatory blood pressure (ABP) were used to monitor patients′ blood pressure. The attainment rates of different monitoring methods were compared. The consistency of the three methods was analyzed. Results Attainment rate of ABP was higher than that of CBP and HBP［35.5%（66/186） vs 20.4%（38/186）, 16.1%（30/186）］（both P＜0.05）. Attainment rate of CBP was consistent with that of ABP (Kappa=0.151, P=0.001), attainment rate of HBP was consistent with that of ABP (Kappa=0.194，P＜0.00), but attainment rate of CBP was not consistent with that of HBP (Kappa=0.043, P=0.283). Conclusion Among different blood pressure monitoring methods in patients with hypertension in plateau area, ABP has the highest attainment rate, which is consistent with that of CBP and HBP at a certain degree.
