• 目的 探讨静脉用药集中调配中心（PIVAS）住院患者退药情况及对策。方法 选取中国人民解放军联勤保障部队第九〇四医院PIVAS 2019年1—12月收集的药物医嘱2 076 485条。记录退药医嘱条数，计算不同退药种类、退药科室以及退药原因的退药率。结果 2 076 485条医嘱中，共有退药医嘱66 350条，退药率为3.2%。退药种类前3位分别为抗菌药物［5.2%（21 207/405 367）］、辅助药物［3.8%（13 627/355 490）］及心脑血管系统药物［3.3%（10 888/332 504）］；退药率前3位的科室分别为神经外科［4.8%（23 323/486 237）］、普外科［4.7%（14 829/312 684）］及骨科［4.7%（9 630/206 019）］；退药原因前3位分别为患者出院［3.7%（22 948/622 409）］、治疗方案改变［3.6%（19 550/548 100）］以及转科［3.3%（13 105/400 550）］。结论 目前本院中住院患者的PIVAS退药率较高，应尽快实施管理措施进行针对性改进，降低住院患者的PIVAS退药率。【

• Objective To investigate the situation and countermeasures of drug withdrawal of inpatients in pharmacy intravenous admixture service (PIVAS). Methods From January to December 2019, 2 076 485 medical orders in the PIVAS in the 904th Hospital of Joint Logistics Support Force of the Chinese People′s Liberation Army were selected. The number of drug withdrawal medical orders was recorded, and the drug withdrawal rates of the different types, departments and reasons of drug withdrawal were calculated. Results Among 2 076 485 medical orders, there were 66 350 drug withdrawal orders, drug withdrawal rate was 3.2%. The top three types of drug withdrawal were antibacterial drugs［5.2%（21 207/405 367）］, adjuvant drugs［3.8%（13 627/355 490）］and cardiocerebral vascular system drugs［3.3%（10 888/332 504）］, respectively. The top three departments of drug withdrawal were departments of neurosurgery［4.8%（23 323/486 237）］, general surgery［4.7%（14 829/312 684）］ and orthopedics［4.7%（9 630/206 019）］, respectively. The top three reasons of drug withdrawal were discharge［3.7%（22 948/622 409）］, change of treatment plan［3.6%（19 550/548 100）］, and transfer of department［3.3%（13 105/400 550）］, respectively. Conclusions  At present, the rate of drug withdrawal is high among inpatients in PIVAS of our hospital. Management measures should be implemented as soon as possible for targeted improvement to reduce the drug withdrawal rate of PIVAS in inpatients.
