• 摘要：

• 目的 探索三级肿瘤专科医院基于疾病诊断相关分组（DRGs）评价指标应用综合指数法对医疗质量评价的实践及效果，为三级肿瘤专科医院质量管理提供新思路。方法 本研究依托上海联众DRGs数据分析平台结合医院病案首页应用综合指数法，从业务能力、学科水平、疑难诊治程度、医疗服务效率及医疗安全5个维度对医院外科系统、内科系统和放疗系统各科室医疗质量进行分析评价。结果 DRGs按综合指数法评价医疗质量排名E、A、B、T、Q、U、Y科室在各诊疗系统内医疗质量好于其他科室，综合得分分别为1.314、1.158、0.966、1.509、1.034、1.609、1.265；D、I、J、S、W科室在各诊疗系统排名靠后，综合得分分别为0.582、0.569、0.432、0.879、0.913，说明科室医疗质量各维度出现不同程度的问题。结论 基于DRGs评价指标应用综合指数法能够客观评价各科室医疗质量，准确地反映各科室医疗质量的优势和不足，也可依据评价结果对各科室存在的主要问题制定针对性医疗质量改进措施，提高医、护、患三方满意度。

• Objective To explore the practice and effect of evaluation of comprehensive index method on medical quality based on disease diagnosis related groups(DRGs) evaluation indexs, providing new ideas for the quality management of tertiary tumor specialized hospitals. Methods Based on the Shanghai Lianzhong DRGs data analysis platform and the hospital medical record homepage application comprehensive index method, this study analyzed and evaluated the medical quality of hospital surgical system, internal medicine system and radiotherapy system departments from business ability, subject level, difficulty diagnosis and treatment, medical service efficiency and medical safety. Results After DRGs evaluating the medical quality ranking according to the comprehensive index method, the medical quality of the E, A, B, T, Q, U and Y departments in each medical treatment system was better than that of other departments, and the comprehensive scores were 1.314, 1.158, 0.966, 1.509, 1.034, 1.609, 1.265; D, I, J, S and W departments in each diagnosis and treatment system at the bottom of the rankings, the comprehensive scores were 0.582, 0.569, 0.432, 0.879, 0.913; there were different levels of problems in the dimensions of medical quality in the department. Conclusion  Based on the DRGs evaluation indexs, the comprehensive index method can objectively evaluate the medical quality of each department, accurately reflect the advantages and disadvantages of the medical quality of each department, also formulate targeted medical quality improvement measures based on the evaluation Results for the main problems existing in each department and improve the satisfaction of doctors, nurses and patients.
