目的 探讨胆道闭锁患儿肝组织中白细胞介素32(IL-32)的表达水平及与肝纤维化的关系。方法 选取海南省人民医院2017年2月至2020年6月收治的35例胆道闭锁患儿及14例胆管扩张患儿作为观察组和对照组。比较2组肝组织中IL-32 mRNA和蛋白表达及血清肝纤维化指标水平。分析观察组患儿IL-32表达水平与肝纤维化指标水平的相关性。结果 观察组患儿肝组织中IL-32 mRNA和蛋白水平均明显高于对照组[(0.054±0.003)比(0.021±0.002)、(0.58±0.04)比(0.40±0.04)],差异均有统计学意义(均P<0.05)。观察组患儿血清肝纤维化指标Ⅲ型前胶原(PCⅢ)、Ⅳ型胶原(Ⅳ-C)、层粘连蛋白(LN)、透明质酸酶(HA)水平均明显高于对照组[(188±23)μg/L比(141±19)μg/L、(161±23)μg/L比(100±12)μg/L、(218±31)μg/L比(143±20)μg/L、(194±25)μg/L比(128±15)μg/L],差异均有统计学意义(均P<0.001)。观察组患儿肝组织中IL-32 mRNA和蛋白表达水平与肝纤维化指标PCⅢ、Ⅳ-C、LN、HA水平均呈正相关(均P<0.05)。结论 胆道闭锁患儿肝组织中IL-32表达水平明显升高,且高表达的IL-32与肝纤维化有一定的相关性。
Objective To investigate the expression of interleukin-32 (IL-32) in liver tissue of children with biliary atresia and its relationship with liver fibrosis. Methods From February 2017 to June 2020, 35 children with biliary atresia and 14 children with biliary dilatation admitted to Hainan General Hospital were selected as the observation group and the control group, respectively. The levels of IL-32 mRNA and protein in liver tissue and serum fibrosis indexes levels were compared between the two groups. The correlation between IL-32 level and liver fibrosis indexes level in the observation group was analyzed. Results The levels of IL-32 mRNA and protein in the liver tissue of the observation group were significantly higher than those of the control group[(0.054±0.003) vs (0.021±0.002), (0.58±0.04) vs (0.40±0.04)](both P<0.05). The serum levels of liver fibrosis index procollagen Ⅲ(PCⅢ), collagen Ⅳ(Ⅳ-C), laminin(LN) and hyaluronidase(HA) of the observation group were significantly higher than those of the control group[(188±23)μg/L vs (141±19)μg/L, (161±23)μg/L vs (100±12)μg/L, (218±31)μg/L vs (143±20)μg/L, (194±25)μg/L vs (128±15)μg/L](all P<0.001). The expression levels of IL-32 mRNA and protein in liver tissue of observation group were positively correlated with liver fibrosis indexes PCⅢ, Ⅳ-C, LN and HA(all P<0.05). Conclusion The expression level of IL-32 in liver tissue of children with biliary atresia significantly increases, and the high expression of IL-32 is related to liver fibrosis.
