中药干预糖尿病肾病肾小管损伤的研究进

• 摘要：

• 糖尿病肾病(DN)是糖尿病最严重的慢性并发症之一。肾小管损伤作为DN发病的关键环节，近年来被证明比肾小球异常出现得更早，在DN病情恶化至终末期肾脏疾病的进程中起着更为重要的作用。中医药治疗DN肾小管损伤疗效确切，可通过调节视黄醇结合蛋白4、中性粒细胞明胶酶相关脂质运载蛋白、肾损伤分子1和肝型脂肪酸结合蛋白等DN新型肾小管损伤诊断标志物以及抑制肾小管上皮细胞转分化、减轻炎症反应、拮抗氧化应激、缓解内质网应激和改善线粒体功能障碍等方面治疗DN肾小管损伤，具有多途径、多靶点的优势。因此，本文对近年来中药治疗DN肾小管损伤的研究进行总结与探讨，为中医药防治DN提供理论参考。

• Diabetic nephropathy (DN) is one of the most serious chronic complications of diabetes mellitus. Kidney tubule injury, as a key link in the pathogenesis of DN, has been proved to occur earlier than glomerular abnormalities in recent years, and plays a more important role in the progression of DN disease deterioration to end-stage kidney disease. Traditional Chinese medicine is effective in treating kidney tubule injury in DN, through regulating new diagnostic markers of kidney tubule injury in DN such as retinol-binding proteins, neutrophil gelatinase associated lipocalin, kidney injury molecule-1 and liver-fatty acid binding protein, as well as inhibiting transdifferentiation of kidney tubule epithelial cells, alleviating inflammatory response, antagonizing oxidative stress, alleviating endoplasmic reticulum stress and improving mitochondrial dysfunction. It has advantages of multiple approaches and multiple targets. Therefore, this paper summarizes and discusses the researches on the treatment of kidney tubule injury in DN with traditional Chinese medicine in recent years, and provides theoretical reference for the prevention and treatment of DN with traditional Chinese medicine.
