不孕患者血清和卵泡液中抗米勒管激素及白细胞介素6与卵巢过度刺激综合征的相关性研究

• 摘要：

• 目的  探讨不孕患者血清和卵泡液中抗米勒管激素（AMH）及白细胞介素6（IL-6）与卵巢过度刺激综合征（OHSS）的相关性。方法  回顾性选取2020年1月至2022年12月济宁医学院附属医院收治的150例接受辅助生殖技术治疗的不孕女性患者。根据是否发生OHSS将患者分为OHSS组（43例）和非OHSS组（107例）。比较2组患者血清性激素、血清和卵泡液中AMH及IL-6水平，使用Pearson法分析血清和卵泡液AMH、IL-6与OHSS的相关性。结果  OHSS组血清雌二醇水平显著高于非OHSS组（P＜0.05），而黄体生成素水平明显低于非OHSS组（P＜0.05）；OHSS组患者血清和卵泡液中AMH、IL-6水平均显著高于非OHSS组［血清：（6.1±1.8）μg/L比（3.3±1.0）μg/L、（4.0±1.1）ng/L比（2.5±0.8）ng/L；卵泡液：（7.4±2.2）μg/L比（4.2±1.2）μg/L、（23.9±7.0）ng/L比（11.7±3.3）ng/L］（均P＜0.001）。Pearson分析结果显示，血清AMH和卵泡液AMH、IL-6水平与OHSS严重程度呈明显正相关（均P＜0.05）。结论  不孕患者血清AMH和卵泡液AMH、IL-6水平与OHSS发生及严重程度密切相关，建议对辅助生殖技术治疗者予以密切监测。

• Objective  To investigate the correlation of anti-Müllerian hormone (AMH) and interleukin-6 (IL-6) in serum and follicular of patients with infertility fluid with ovarian hyperstimulation syndrome (OHSS). Methods  From January 2020 to December 2022, 150 infertile women treated with assisted reproductive technology in the Affiliated Hospital of Jining Medical University were retrospectively selected. The patients were divided into OHSS group (43 cases) and non-OHSS group (107 cases) according to whether OHSS occurred. The levels of serum sex hormones, AMH and IL-6 in serum and follicular fluid were compared between the two groups. Pearson method was used to analyze the correlation between AMH, IL-6 in serum and follicular fluid and OHSS. Results  The level of serum estradiol in OHSS group was significantly higher than that in non-OHSS group (P<0.05), while the level of luteinizing hormone was significantly lower than that in non-OHSS group (P<0.05). The levels of AMH and IL-6 in serum and follicular fluid in OHSS group were significantly higher than those in non-OHSS group ［serum: (6.1±1.8)μg/L vs (3.3±1.0)μg/L, (4.0±1.1)ng/L vs (2.5±0.8)ng/L; follicular fluid: (7.4±2.2)μg/L vs (4.2±1.2)μg/L, (23.9±7.0)ng/L vs (11.7±3.3)ng/L］(both P<0.001). Pearson analysis showed that serum AMH, follicular fluid AMH and IL-6 levels were positively correlated with the severity of OHSS (all P<0.05). Conclusion  The levels of serum AMH and follicular fluid AMH and IL-6 of patients with infertility are closely related to the occurrence and severity of OHSS. It is suggested that patients treated with assisted reproductive technology should be closely monitored.
