• 摘要：

• 左西孟旦是一种新型钙离子增敏剂，具有正性肌力和扩张血管作用；还具有器官保护多效性作用，包括在心脏以外脏器，如肾脏、肝脏、肠道和脾脏的血管床，以及在肺和/或呼吸肌中发挥抗炎、抗氧化和抗凋亡作用。左西孟旦可结合于肌肉（包括膈肌和骨骼肌）中慢肌纤维上饱和的钙离子与肌钙蛋白C，从而增强肌肉的收缩力。目前，左西孟旦越来越多被运用于呼吸系统疾病。本文旨在综述左西孟旦在慢性阻塞性肺疾病伴右心衰竭、肺动脉高压伴右心衰竭、肺栓塞伴右心衰竭、急性呼吸窘迫综合征、脱机困难、脓毒症和脓毒性休克中的研究进展。

• Levosimendan is a new calcium ion sensitizer with positive muscle force and vascular dilation effect. It also has pleiotropic effect of organ protection, including anti-inflammatory, antioxidant and anti-apoptotic effects in organs other than the heart, such as the blood vessel beds of kidney, liver, intestine and spleen, as well as in the lungs and/or respiratory muscles. Levosimendan can combine with calcium ion and troponin C saturated on slow muscle fibers in muscles (including diaphragm and skeletal muscle) to enhance muscle contractility. At present, levosimendan is increasingly used in respiratory system diseases. This paper aims to review the research progress of levosimendan in chronic obstructive pulmonary disease with right heart failure, pulmonary hypertension with right heart failure, pulmonary embolism with right heart failure, acute respiratory distress syndrome, difficulty in weaning, sepsis and septic shock.
