心房颤动个体心房结构重构机制及中医药干预的动物实验研究进展

• 摘要：

• 心房颤动是一种室上性快速性心律失常，以高频心房电活动、不同步心房收缩和不规律心室兴奋为特征，主要表现为心悸、胸闷、乏力、运动耐力下降。随着年龄的增长，其发病率不断提高。心房结构重构与心房颤动的发生密切相关，心肌纤维化是心房结构重构的标志。目前，心房颤动的药物治疗及导管消融均存在一定的局限性，而中医药在改善心房颤动心肌纤维化方面具有独特优势。经文献查阅，发现中药可能通过抗炎、抑制氧化应激、调节核因子κB、磷脂酰肌醇-3-激酶/蛋白激酶B、转化生长因子β1/Smad信号通路、调控基质金属蛋白酶及基质金属蛋白酶抑制剂水平发挥其改善心房颤动心肌纤维化的作用。现对心房颤动心房结构重构机制及中药单体、中药复方有效成分干预心房颤动心房结构重构的动物实验研究进行综述，以期为心房颤动提供新的治疗思路。

• Atrial fibrillation is a supraventricular tachyarrhythmia characterized by high frequency atrial electrical activity, asynchronous atrial contraction, and irregular ventricular excitation. The main manifestations are palpitations, chest tightness, fatigue, and decreased exercise tolerance. Its incidence increases with age. Atrial structural remodeling is closely related to the occurrence of atrial fibrillation, and myocardial fibrosis is a hallmark of atrial structural remodeling. At present, there are certain limitations in the drug treatment and catheter ablation of atrial fibrillation, and traditional Chinese medicine has unique advantages in improving myocardial fibrosis of atrial fibrillation. Through literature review, it was found that traditional Chinese medicine may play a role in improving atrial fibrillation and myocardial fibrosis through anti-inflammatory, inhibition of oxidative stress, regulation of nuclear factor-κB, phosphatidylinositol-3-kinase/protein kinase B, transforming growth factor-β1/Smad signaling pathway, regulation of matrix metalloproteinase and matrix metalloproteinases inhibitor levels. This article reviews the mechanism of atrial structural remodeling in atrial fibrillation and the animal experimental research on the intervention of Chinese medicine monomer and Chinese medicine compound active ingredients in atrial structural remodeling of atrial fibrillation, in order to provide new ideas for the treatment of atrial fibrillation.
