解郁安神颗粒联合奥氮平治疗精神分裂症伴轻度焦虑抑郁患者的临床研究

• 摘要：

• 目的  探讨解郁安神颗粒联合奥氮平治疗精神分裂症伴轻度焦虑、抑郁患者的临床疗效。方法  选取2020年1月至2022年12月在青海省第三人民医院接受治疗的102例精神分裂症伴轻度焦虑、抑郁患者，将其完全随机分为观察组（51例）和对照组（51例）。在常规心理疏导基础上，对照组患者采用奥氮平片进行治疗，观察组患者在对照组基础上联用解郁安神颗粒进行治疗。比较2组患者治疗2个月后的总有效率。记录并比较2组患者治疗前后的阳性与阴性症状量表（PANSS）、认知功能评定量表（LOTCA）、焦虑自评量表（SAS）和抑郁自评量表（SDS）评分。记录并比较2组患者出现的不良反应。结果  观察组总有效率高于对照组［88.2%（45/51）比70.6%（36/51）］，差异有统计学意义（P＜0.05）。治疗后，2组患者PANSS、SAS以及SDS评分均明显低于治疗前，LOTCA评分明显高于治疗前，且观察组患者的PANSS、SAS以及SDS评分低于对照组，LOTCA评分高于对照组（均P＜0.05）。治疗后2组患者血清褪黑素、5-羟色胺水平均明显高于治疗前，且观察组均高于对照组（均P＜0.05）。观察组与对照组不良反应发生率比较［15.7%(8/51)比23.5%(12/51)］，差异无统计学意义（P=0.318）。结论  解郁安神颗粒联合奥氮平治疗精神分裂症伴轻度焦虑、抑郁患者具有较好的疗效，可改善患者的精神状态、认知功能，减轻焦虑、抑郁程度，且安全性较好。

• Objective  To explore the clinical efficacy of Jieyu Anshen granule combined with olanzapine in the treatment of schizophrenia with mild anxiety and depression. Methods  A total of 102 schizophrenia patients with mild anxiety and depression who were treated in the Third People′s Hospital of Qinghai Province from January 2020 to December 2022 were selected. They were randomly divided into observation group (51 cases) and control group (51 cases). On the basis of routine psychological counseling, the control group was treated with olanzapine tablets, and the observation group was treated with Jieyu Anshen granules on the basis of the control group. The total effective rate was compared between the two groups after 2 months of treatment. The positive and negative syndrome scale (PANSS) score, Loewenstein occupational therapy cognitive assessment (LOTCA) score, self-rating anxiety scale (SAS) score and self-rating depression scale (SDS) score before and after treatment were recorded and compared between the two groups. The adverse reactions of the two groups were recorded and compared. Results  The total effective rate of the observation group was higher than that of the control group ［88.2%(45/51) vs 70.6%(36/51)］ (P＜0.05). After treatment, the PANSS score, SAS score and SDS score of the two groups were significantly lower than those before treatment, and the LOTCA score was significantly higher than that before treatment. The PANSS score, SAS score and SDS score of the observation group were lower than those of the control group, and the LOTCA score was higher than that of the control group (all P＜0.05). After treatment, the levels of serum melatonin and 5-hydroxytryptamine in the two groups were significantly higher than those before treatment, and those in the observation group were higher than those in the control group (all P＜0.05). The incidence of adverse reactions was compared between the observation group and the control group［15.7%(8/51) vs 23.5%(12/51)］ (P=0.318). Conclusion  Jieyu Anshen granule combined with olanzapine in the treatment of schizophrenia with mild anxiety and depression has a good effect, which can improve the mental state and cognitive function of patients, reduce the degree of anxiety and depression, and the safety is good.
