• 摘要：

• 目的 分析射频消融术联合泡沫硬化剂注射对下肢静脉曲张患者生活质量及术后并发症的影响。方法 选取2022年1月至2023年12月西北大学附属医院·西安市第三医院收治的下肢静脉曲张患者100例作为研究对象。随机（红白球）分为对照组（50例）和观察组（50例），对照组实施传统高位结扎+点状抽剥治疗，观察组实施腔内射频消融术联合泡沫硬化剂注射治疗。对比2组治疗效果、手术指标、疼痛应激水平、血流动力学、凝血功能指标及并发症发生率。结果观察组总有效率高于对照组［98.0%（49/50）比82.0%（41/50）］，差异有统计学意义（P＜0.05）。观察组手术时间、住院时间均短于对照组，术中出血量少于对照组，差异均有统计学意义（均P＜0.05）。治疗后观察组视觉模拟量表评分、缓激肽、P物质水平均低于对照组，血流动力学及凝血功能较对照组改善更明显，慢性静脉功能不全问卷14评分低于对照组（均P＜0.05）。观察组并发症发生率低于对照组［2.0%（1/50）比12.0%（6/50）］，差异有统计学意义（P＜0.05）。结论 射频消融术联合泡沫硬化剂注射治疗下肢静脉曲张能够减轻患者的疼痛应激情况，改善其血流动力学和凝血功能及生活质量，应用价值较高。

• Objective To analyze the effect of radiofrequency ablation combined with foam sclerotherapy injection on the quality of life and postoperative complications in patients with lower extremity varicose veins. Methods A total of 100 patients with lower extremity varicose veins admitted to the Affiliated Hospital of Northwest University, Xi′an No.3 Hospital from January 2022 to December 2023 were selected as the research objects. The patients were randomly (red and white ball) divided into control group (50 cases) and observation group (50 cases). The control group was treated with traditional high ligation and punctate stripping, and the observation group was treated with intracavitary radiofrequency ablation combined with foam sclerotherapy injection. The therapeutic effects, surgical indicators, pain stress level, hemodynamics, coagulation function indicators and the incidence of complications were compared between the two groups. Results The total effective rate of the observation group was higher than that of the control group ［98.0% (49/50) vs 82.0%(41/50)］(P<0.05). The operation time and hospitalization time of the observation group were shorter than those of the control group, and the intraoperative blood loss was less than that of the control group (all P<0.05). After treatment, the visual analogue scale score, the levels of bradykinin and substance P in the observation group were lower than those in the control group, the hemodynamics and coagulation function were improved more significantly than those in the control group, and the score of chronic venous insufficiency questionnaire 14 was lower than that in the control group (all P<0.05). The incidence of complications in the observation group was lower than that in the control group ［2.0%(1/50) vs 12.0%(6/50)］(P<0.05). Conclusions Radiofrequency ablation combined with foam sclerotherapy injection in the treatment of lower extremity varicose veins can reduce the pain and stress of patients, improve their hemodynamics, coagulation function and quality of life, and has high application value.
