经皮弯角椎体成形术治疗骨质疏松性椎体压缩骨折的临床效果研究

• 摘要：

• 目的 探讨经皮弯角椎体成形术（PCVP）治疗骨质疏松性椎体压缩骨折(OVCF)的临床效果。方法 收集2020年1月至2023年6月在民航总医院骨科因OVCF行手术治疗的230例患者的临床资料进行回顾性分析。根据手术方法的不同分为对照组（120例）和观察组（110例）。对照组采用传统单侧经皮椎体成形术，观察组采用PCVP。比较2组患者骨水泥用量及骨水泥分布情况，术前及术后24 h、术后3、6个月腰背部疼痛视觉模拟量表（VAS）评分、Oswestry功能障碍指数（ODI）评分。结果 观察组患者术中骨水泥用量、骨水泥分布优秀率高于对照组［（4.7±0.7）ml比（3.4±0.7）ml、81.8%（90/110）比57.5%（69/120）］，差异均有统计学意义（均P<0.01）。2组患者术后24 h及术后3、6个月VAS及ODI评分均显著低于术前（均P＜0.05），但术后2组间差异均无统计学意义（均P＞0.05）。结论 2种手术方法治疗OVCF均能迅速缓解患者疼痛，改善患者生活质量，疗效可靠，PCVP与传统手术疗效相近；PCVP骨水泥用量更多，骨水泥分布更好，在骨性支撑方面表现更佳。

• Objective  To investigate the clinical effect of percutaneous curved vertebroplasty (PCVP) in the treatment of osteoporotic vertebral compression fractures (OVCF). Methods The clinical data of 230 patients with OVCF who underwent surgical treatment in the Department of Orthopedics, Civil Aviation General Hospital from January 2020 to June 2023 were collected and retrospectively analyzed.According to the different surgical methods , they were divided into the control group (120 cases) and the observation group (110 cases). The control group was treated with traditional unilateral percutaneous vertebroplasty, and the observation group was treated with PCVP.The dosage and distribution of bone cement, visual analogue scale (VAS) scores of low back pain and Oswestry disability index (ODI) scores before operation and at 24 hours, 3 months and 6 months after operation were compared between the two groups. Results The amount of bone cement and the excellent rate of bone cement distribution in the observation group were higher than those in the control group［(4.7±0.7)ml vs (3.4±0.7)ml, 81.8%(90/110) vs 57.5%(69/120)］(both P<0.01). The VAS and ODI scores of the two groups at 24 hours, 3 months and 6 months after operation were significantly lower than those before operation(all P<0.05). However, there was no significant difference between the two groups after operation (all P>0.05). Conclusions  The two surgical methods  in the treatment of OVCF can quickly relieve pain, improve the quality of life of patients, and the curative effect is reliable. PCVP and traditional surgery have similar efficacy. PCVP consumes more bone cement, has better bone cement distribution, and performs better in bone support.
