安神补心六味丸联合氯吡格雷治疗冠心病心绞痛的临床效果观察

• 摘要：

• 目的 观察安神补心六味丸联合氯吡格雷对冠心病心绞痛（CHDA）患者的治疗效果。方法 选择2023年2月至2024年1月黑龙江省第二医院收治的CHDA患者105例，采用随机数字表法分为对照组（52例）和观察组（53例）。对照组在常规治疗基础上应用氯吡格雷治疗，观察组在对照组基础上给予安神补心六味丸治疗，2组均治疗1个月。治疗后进行临床疗效评价，同时比较2组治疗前后的心绞痛干预和发作情况、心功能指标、血液流变学指标、血管内皮功能指标及治疗期间的不良反应发生率。结果 观察组治疗总有效率高于对照组［96.2%（51/53）比82.7%（43/52）］，差异有统计学意义（χ2=5.126，P=0.024）。治疗后2组硝酸甘油单次用量、心绞痛发作次数及持续时间、左心室收缩末期内径、血浆黏度、全血高切黏度及低切黏度、内皮素水平少于/短于/小于/低于治疗前且观察组少于/短于/小于/低于对照组，左心室射血分数、心输出量和一氧化氮水平大于/高于治疗前且观察组大于/高于对照组，差异均有统计学意义（均P<0.05）。治疗期间观察组和对照组不良反应发生率比较差异无统计学意义［3.8%（2/53）比5.8%（3/52）］（χ2=0.191，P=0.663）。结论 安神补心六味丸联合氯吡格雷治疗CHDA的临床效果较好，能够明显减少心绞痛发作，改善患者心功能、血液流变学和血管内皮功能，且安全性较好。

• Objective To observe the therapeutic effect of Anshen Buxin Liuwei pills combined with clopidogrel on patients with coronary atherosclerotic heart disease and angina pectoris(CHDA). Methods A total of 105 CHDA patients admitted to the Second Hospital of Heilongjiang Province from February 2023 to January 2024 were selected. According to the random number table method, they were divided into the control group (52 cases) and the observation group (53 cases). The control group was treated with clopidogrel on the basis of routine treatment, and the observation group was treated with Anshen Buxin Liuwei pills on the basis of the control group. Both groups were treated for 1 month. The clinical efficacy was evaluated after treatment, and the intervention and occurrence of angina pectoris, cardiac function index, hemorheology index, vascular endothelial function index before and after treatment and the incidence of adverse reactions during treatment were compared between the two groups. Results The total effective rate of the observation group was higher than that of the control group ［96.2%(51/53) vs 82.7%(43/52)］(χ2=5.126, P=0.024). After treatment, the single dosage of nitroglycerin, the number and duration of angina pectoris attack, left ventricular end-systolic diameter, plasma viscosity, whole blood high shear viscosity and low shear viscosity, and endothelin level in the two groups were less/shorter/smaller/lower than those before treatment, those in the observation group were less/shorter/smaller/lower than those in the control group, the left ventricular ejection fraction, cardiac output and nitric oxide levels were greater/higher than those before treatment, and those in the observation group were greater/higher than those in the control group (all P<0.05). There was no significant difference in the incidence of adverse reactions between the observation group and the control group during the treatment ［3.8%(2/53) vs 5.8%(3/52)］（χ2=0.191, P=0.663). Conclusions  Anshen Buxin Liuwei pills combined with clopidogrel has good clinical effect in the treatment of CHDA, which can significantly reduce the incidence of angina pectoris, improve the cardiac function, hemorheology and vascular endothelial function of patients, and has good safety.
