术中体温变化与术后认知功能障碍的关系概述

• 摘要：

• 术后认知功能障碍（POCD）是老年人术后主要并发症之一。许多研究报道了POCD的危险因素，其中围手术期低体温既是手术常见危险因素，也是诱发POCD的高危因素。低体温会导致围手术期出血量的增加，从而增加了感染概率，尤其在泌尿外科手术中，术中使用大量低温冲洗液，常常导致患者于术中、术后出现低体温。这将极大地影响患者术后恢复，并延长住院时间。如何解决术中出现低体温问题及降低POCD的发生率，目前仍没有满意的解决方法。本文阐述POCD的研究现状及术中体温变化尤其是术中低体温与POCD的关系，提示术中体温保护的重要性，预期可通过良好的体温保护策略改善患者预后。

• Postoperative cognitive dysfunction (POCD) is one of the major postoperative complications in the elderly. Many studies have reported the risk factors of POCD, among which perioperative hypothermia is not only a common risk factor for surgery, but also a high risk factor for inducing POCD. Hypothermia can lead to an increase in perioperative blood loss, which increases the probability of infection. Especially in urology surgery, the use of a large amount of low temperature irrigation fluid during the operation, patients often have hypothermia during and after the operation. This will greatly affect the patient′s postoperative recovery and prolong the length of hospital stay. There is still no satisfactory solution to solve the problem of intraoperative hypothermia and reduce the incidence of POCD. This article describes the research status of POCD and the changes of intraoperative body temperature, especially the relationship between intraoperative hypothermia and POCD, suggesting the importance of intraoperative body temperature protection, and it is expected to improve the prognosis of patients through a good body temperature protection strategy.
