• 摘要：

• 代谢相关脂肪性肝病（MAFLD）作为一种日益严峻的全球性健康疾病，其发病率不断攀升且年轻化，严重危害人类健康，并给社会发展带来沉重的经济负担。近年来发现，肝细胞衰老在促进肝脏脂质沉积、加剧肝损伤以及引发脂质代谢紊乱中扮演着不容忽视的角色。目前，针对肝细胞衰老影响MAFLD病程的深入机制及药物干预研究的总结性综述尚显不足，临床治疗策略发展也相对滞后。本文从调控自噬、DNA甲基化改变、铁死亡、对肝脏再生影响及药物机制研究的角度，重点梳理总结肝细胞衰老在MAFLD中调控机制的研究进展，以期为深入的基础研究提供参考依据，并为MAFLD的临床治疗和药物开发提供新的视角和实践指导。

• As an increasingly serious global health disease, metabolic associated fatty liver disease (MAFLD) has a rising incidence in young people, which seriously endangers human health and brings heavy economic burden to social development. In recent years, it has been found that hepatocyte senescence plays a non-negligible role in promoting liver lipid deposition, aggravating liver injury, and causing lipid metabolism disorders. At present, a summary review on the in-depth mechanism and drug intervention of hepatocyte senescence affecting the course of MAFLD is still insufficient, and the development of clinical treatment strategies is relatively lagging behind. This article focuses on the research progress of the regulatory mechanism of hepatocyte senescence in MAFLD from the perspective of the regulation of autophagy, DNA methylation changes, ferroptosis, effects on liver regeneration and drug mechanisms. This article aims to provide a reference for in-depth basic research, and provide a new perspective and practical guidance for the clinical treatment and drug development of MAFLD.
