北京市某三甲综合医院急性弛缓性麻痹病例智能监测预警效果评价

  • 目的 评价北京市某三甲综合医院急性弛缓性麻痹(AFP)病例智能监测预警系统效果,为同类医疗机构提供参考。方法 以首都医科大学附属北京安贞医院为例,优化前基线资料为2024年1—6月AFP监测预警数据,优化后结局资料为2025年1—6月AFP监测预警数据。比较其AFP病例监测预警系统优化前后,监测预警弹窗数量、病种、触发预警率及管理流程方面的差异。结果 系统优化后,临床医生诊断病种中含“神经、麻痹、脊髓”等14种AFP疾病关键词即可实时触发弹窗;2025年1—6月临床医生触发弹窗113条,较2024年同期增加16.49%,涉及病种从2种增加至7种。2025年1—6月<15岁患者就诊量较2024年同期下降22.92%,弹窗触发预警率2.97‰,显著高于2024年同期1.96‰(U=2.991,P<0.01)。此外,职能管理科室弹窗界面新增预警触发原因、处置依据及结果记录功能,同步增设诊断模糊字段检索功能,支持实时检索全院各科室任意诊断关键词。结论 系统优化后,AFP病例弹窗触发灵敏度显著提升,有助于提高临床医生对AFP病例的识别与报告意识;职能管理科室可实现处置记录永久追溯。诊断模糊字段检索功能,既能减少临床医生无关弹窗、减轻工作负担,又提升检索效率,还为各类传染病监测提供灵活的关键词检索渠道。

  • Objective To evaluate the effect of the monitoring and early warning system for acute flaccid paralysis (AFP) cases in a tertiary general hospital in Beijing, and to provide a reference for similar medical institutions. Methods Taking Beijing Anzhen Hospital, Capital Medical University as an example, the baseline data before optimization were the AFP monitoring and early warning data from January to June 2024, and the outcome data after optimization were the AFP monitoring and early warning data from January to June 2025. The differences in the number of monitoring and early warning pop-ups, disease types, early warning trigger rate and management process of the AFP case monitoring and early warning system before and after optimization were compared. Results After the system optimization, real-time pop-ups could be triggered when clinical doctors included 14 AFP disease keywords such as "nerve, paralysis, spinal cord" in the diagnosed disease types. A total of 113 pop-ups were triggered by clinical doctors from January to June 2025, an increase of 16.49% compared with the same period in 2024, involving an increase in disease types from 2 to 7. The number of visits by patients under 15 years old from January to June 2025 decreased by 22.92% compared with the same period in 2024, and the pop-up early warning trigger rate was 2.97‰, which was significantly higher than 1.96‰ in the same period in 2024 (U=2.991, P<0.01). In addition, the pop-up interface of the functional management department added the functions of recording the cause of early warning trigger, disposal basis and results , and simultaneously added the retrieval function of fuzzy diagnosis fields, supporting real-time retrieval of any diagnosis keywords in all departments of the hospital. Conclusion After the system optimization, the sensitivity of AFP case pop-up trigger is significantly improved, which helps to enhance clinicians′ awareness of AFP case identification and reporting; the functional management department can realize permanent traceability of disposal records. The fuzzy diagnosis field retrieval function can not only reduce irrelevant pop-ups for clinicians and lighten their workload, but also improve retrieval efficiency, and provide a flexible keyword retrieval channel for the monitoring of various infectious diseases.
