重复经颅磁刺激联合复方苁蓉益智胶囊治疗帕金森病合并睡眠障碍的效果及对患者血清相关指标水平的影响

• 摘要：

• 目的 探讨重复经颅磁刺激联合复方苁蓉益智胶囊治疗帕金森病合并睡眠障碍的效果及对患者血清中枢神经特异性蛋白（S-100β）和活化T细胞趋化因子（RANTES）水平的影响。方法 选取2023年10月至2024年2月在河北北方学院附属第一医院接受治疗的88例帕金森病合并睡眠障碍患者，按随机数字表法分为对照组和观察组，各44例。对照组在一般治疗上联合重复经颅磁刺激治疗方案，观察组在对照组基础上联合复方苁蓉益智胶囊治疗。比较2组睡眠质量、生活质量、炎症因子水平、血清S-100β及RANTES水平、临床疗效。结果 治疗后，2组匹兹堡睡眠质量指数量表评分均低于治疗前（均P＜0.05），且观察组低于对照组［（7.5±0.9）分比（10.4±1.3）分］（P＜0.001）；2组统一帕金森病评分量表评分、帕金森病生活质量调查量表评分均低于治疗前，且观察组均低于对照组（均P＜0.05）。治疗后，2组肿瘤坏死因子α、白细胞介素1β水平均低于治疗前，且观察组均低于对照组（均P＜0.05）；2组血清S-100β、RANTES水平均低于治疗前，且观察组均低于对照组（均P＜0.05）。观察组临床总有效率高于对照组［97.7%（43/44）比81.8%（36/44）］（P=0.014）。结论 重复经颅磁刺激联合复方苁蓉益智胶囊治疗帕金森病合并睡眠障碍的效果较好，可改善睡眠质量及生活质量，降低炎症因子水平及血清S-100β、RANTES水平。

• Objective To investigate the efficacy of repetitive transcranial magnetic stimulation (rTMS) combined with compound Congrong Yizhi capsules in the treatment of Parkinson′s disease (PD) complicated with sleep disorders and its effects on serum levels of central nervous system-specific protein (S-100β) and regulated on activation, normal T cell expressed and secreted (RANTES). Methods A total of 88 patients with PD complicated with sleep disorders who were treated in the First Affiliated Hospital of Hebei North University from October 2023 to February 2024 were selected and divided into the control group and the observation group with 44 cases in each group by random number table method. The control group was treated with rTMS on the basis of conventional therapy, and the observation group was treated with compound Congrong Yizhi capsules on the basis of the control group′s regimen. Sleep quality, quality of life, levels of inflammatory factors, serum levels of S-100β and RANTES, and clinical efficacy were compared between the two groups. Results After treatment, the Pittsburgh sleep quality index scores of both groups were lower than those before treatment (all P＜0.05), and the score of the observation group was lower than that of the control group ［(7.5±0.9) vs (10.4±1.3)］(P＜0.001). The unified PD rating scale scores and PD quality of life questionnaire scores of both groups were lower than those before treatment, and the scores of the observation group were lower than those of the control group (all P＜0.05). After treatment, the serum levels of tumor necrosis factor-α and interleukin-1β in both groups were lower than those before treatment, and the levels in the observation group were lower than those in the control group (all P＜0.05). The serum levels of S-100β and RANTES in both groups were lower than those before treatment, and the levels in the observation group were lower than those in the control group (all P＜0.05). The total clinical effective rate of the observation group was higher than that of the control group ［97.7%(43/44) vs 81.8%(36/44)］(P=0.014). Conclusion rTMS combined with compound Congrong Yizhi capsules has a good efficacy in the treatment of PD complicated with sleep disorders, which can improve sleep quality and quality of life, and reduce the levels of inflammatory factors as well as serum S-100β and RANTES.
