• 摘要：

• 慢性心力衰竭作为心血管疾病的终末阶段，其高死亡率、再住院率及医疗负担已成为全球公共卫生挑战。心力衰竭患者出院早期3个月病情多变、危险程度高，是死亡和再入院的高发阶段，对此《中国心力衰竭诊断和治疗指南2018》提出心力衰竭“易损期”这一概念。中医在“治未病”理论指导下，结合证候动态演变特点，构建心力衰竭易损期预警体系，对心力衰竭易损期进行早期风险预警监测，为中西医结合防治提供新思路。

• As the terminal stage of cardiovascular diseases, chronic heart failure (CHF) has become a global public health challenge due to its high mortality, readmission rate and medical burden. The early 3 months after discharge is a period of variable condition and high risk for heart failure patients, with a high incidence of death and readmission. In this regard, the 2018 Chinese guidelines for the diagnosis and treatment of heart failure proposed the concept of the "vulnerable phase" of heart failure. Under the guidance of the traditional Chinese medicine theory of "preventing disease before its onset", combined with the characteristics of dynamic evolution of syndromes, constructing an early warning system for the vulnerable phase of heart failure to conduct early risk warning and monitoring for this phase provides a new idea for the integrated prevention and treatment of traditional Chinese and western medicine.
