• 摘要：

• 心房颤动是最常见的心律失常之一，具有较高的脑卒中和死亡风险。射频消融已成为主要治疗手段，其中肺静脉隔离是核心策略，但在持续性心房颤动中常需联合线性消融。为提高手术质量与安全性，心房颤动射频消融已从经验性操作转向量化消融时代。作为综合反映病变深度与体积的量化指标，消融指数已被证明与电隔离完整性、复发率及并发症密切相关。然而，不同解剖部位的消融指数参数和功率设置尚无统一标准。本综述概述消融指数指导下各部位参数选择的研究进展，为心房颤动射频消融的精准化提供参考。

• Atrial fibrillation is one of the most common arrhythmias with high risks of stroke and mortality. Radiofrequency ablation has become the primary treatment modality, with pulmonary vein isolation as the core strategy, and linear ablation is usually combined for persistent atrial fibrillation. To improve surgical quality and safety, radiofrequency ablation for atrial fibrillation has transitioned from empirical operation to the era of quantitative ablation. As a quantitative indicator comprehensively reflecting lesion depth and volume, ablation index has been confirmed to be closely associated with the integrity of electrical isolation, recurrence rate and complications. However, unified standards for ablation index parameters and power settings at different anatomical sites are still lacking. This review summarizes the research progress of parameter selection at various sites guided by ablation index, so as to provide references for the precise implementation of radiofrequency ablation in atrial fibrillation.
