基于扶正祛邪理论与肠道微生态探讨中医药治疗溃疡性结肠炎的作用机制

• 摘要：

• 溃疡性结肠炎（UC）作为一种慢性、复发性的炎症性肠病，其发病机制与肠道微生态稳态密切相关。肠道微生态失衡会通过破坏肠道黏膜屏障、激活异常免疫反应及代谢紊乱等途径推动UC发生发展。中医扶正祛邪治则在UC治疗中展现出独特优势：扶正类中药通过富集有益菌、调节代谢产物及增强肠屏障功能恢复肠道微生态平衡；祛邪类中药则通过抑制致病菌增殖、减轻炎症反应发挥治疗作用，二者通过协同作用可多靶点、多途径重塑肠道微生态，为UC治疗提供新思路。本文系统综述肠道微生态与UC的关联机制以及扶正祛邪中药对UC肠道微生态的调节作用，旨在为UC的临床治疗及中药机制研究提供理论依据。

• Ulcerative colitis (UC) is a chronic and recurrent inflammatory bowel disease, and its pathogenesis is closely related to the homeostasis of intestinal microecology. Intestinal microecological imbalance promotes the occurrence and progression of UC by damaging the intestinal mucosal barrier, activating abnormal immune responses and inducing metabolic disorders. The traditional Chinese medicine therapeutic principle of strengthening vitality and eliminating pathogens shows unique advantages in the treatment of UC. Vitality-strengthening Chinese medicines can restore intestinal microecological balance by enriching beneficial bacteria, regulating metabolites and enhancing intestinal barrier function. Pathogen-eliminating Chinese medicines exert therapeutic effects by inhibiting the proliferation of pathogenic bacteria and alleviating inflammatory reactions. The synergistic effect of the two can reshape intestinal microecology through multi-target and multi-pathway approaches, providing new ideas for the treatment of UC. This article systematically reviews the correlation mechanism between intestinal microecology and UC, as well as the regulatory effect of traditional Chinese medicines for strengthening vitality and eliminating pathogens on intestinal microecology in UC, aiming to provide theoretical basis for clinical treatment of UC and mechanism research of traditional Chinese medicine.
