• 摘要：

• 糖尿病足溃疡是糖尿病患者常见且顽固的并发症之一，常规治疗手段包括清创、抗感染及减压等措施，但总体愈合率有限、复发率高，且难以有效控制感染与改善长期预后。富血小板血浆因富含多种生长因子和细胞因子，能够改善局部微环境，促进血管新生、胶原沉积及组织修复，近年来逐渐成为慢性创面治疗的重要研究方向。富血小板血浆治疗糖尿病足溃疡的相关研究已从单一应用发展到与抗感染药物、中药方剂、外泌体、干细胞及多种物理疗法的联合使用。联合策略在抗感染、改善血供、调控炎症及加速愈合等方面均较单一治疗具有优势。本文综述了近年该领域的研究进展，并探讨了其在糖尿病足溃疡精准治疗中的潜在价值与挑战。

• Diabetic foot ulcer is one of the common and intractable complications in diabetic patients. Conventional treatments include debridement, anti-infection, offloading and other measures, but the overall healing rate is limited, the recurrence rate is high, and it is difficult to effectively control infection and improve long-term prognosis. Platelet-rich plasma is rich in a variety of growth factors and cytokines, which can improve the local microenvironment, promote angiogenesis, collagen deposition and tissue repair, and has gradually become an important research direction in the treatment of chronic wounds in recent years. Studies on platelet-rich plasma in the treatment of diabetic foot ulcers have evolved from single application to combined use with anti-infective drugs, traditional Chinese medicine prescriptions, exosomes, stem cells and various physical therapies. Combined strategies have advantages over single therapy in anti-infection, improving blood supply, regulating inflammation and accelerating healing. This article reviews the research progress in this field in recent years, and discusses its potential value and challenges in the precise treatment of diabetic foot ulcers.
