大动脉炎重要血管受累的研究进展

• 摘要：

• 大动脉炎是一种罕见的、慢性大血管炎症，累及主动脉及其主要分支并可累及重要脏器血管，如心、脑、肺、肾等。临床表现因血管受累部位和炎症程度而各有不同。主动脉弓上动脉受累影响上肢及脑供血；冠状动脉受累可引发急性冠状动脉综合征；肺动脉受累可发展为肺动脉高压甚至右心衰竭；肾动脉受累可继发难治性高血压。不同脏器受累产生的并发症应注意早期发现、强化内科治疗，结合介入及外科治疗措施以改善重要脏器血供。重视大动脉炎所致重要器官血管受累对改善患者预后具有重要意义，本文就大动脉炎重要血管受累的情况及治疗进展进行综述。

• Takayasu′s arteritis is a rareand  chronic vasculitis that mainly effects the aorta and its major branches and other vessels of important organs such as the heart, brain, lung, kidney, etc. Clinical characteristics of takayasu′s arteritis were changed by the position of vessels and the level of inflammation. Superior arcuate artery of aorta with takayasu′s arteritismay affects the blood flow of upper limbs and brain. Coronary artery with takayasu′s arteritis may cause acute coronary syndrome. Pulmonary artery with takayasu′s arteritis may cause pulmonary artery hypertension or even right heart failure. Renal artery with takayasu′s arteritis  may contribute to secondary refractory hypertension. Complications caused by different organ involvement should be early detected, consolidates medical treatment with interventional therapy and surgical orperation can improve the blood supply of organs. More attention needs to be paid to the important vesselwith takayasu′s arteritis can significantly improve the prognosis of patients. This paper reviewd the situation and treatment progress of important vascular involvement in takayasu′s arteritis.
