  • 代谢综合征是一组以胰岛素抵抗、超重/肥胖为核心,合并糖脂代谢异常、高血压等的代谢失衡综合征。心外膜脂肪组织作为一种特殊的脂肪组织,既具备脂肪储能、机械保护的生理特性;又具有高分泌活性,能够释放多种抗炎/炎症因子,调节游离脂肪酸的摄取/释放,其代谢稳态在代谢综合征的进程中发挥着重要的作用。心外膜脂肪组织与代谢综合征各组分的相关性研究也不断取得新进展,主要从异位脂肪堆积、游离脂肪酸超载、脂肪低度炎症、氧化应激方面探讨影响代谢综合征的核心机制,通过改善生活方式、药物干预、手术干预等措施均可减少心外膜脂肪组织体积,进而改善代谢综合征,减少心血管事件。因此本文将心外膜脂肪组织与代谢综合征的相关性、影响机制及治疗策略研究进展进行综述。

  • Metabolic syndrome is characterized by insulin resistance, obesity/overweight, and abnormalities in glucose and lipid metabolism, as well as hypertension. Epicardial adipose tissue (EAT) is a specialized type of adipose tissue with both the physiological function of energy storage and mechanical protection, and high secretory activity that releases various anti-inflammatory/inflammatory factors while regulating the intake/release of free fatty acids. It plays a crucial role in maintaining metabolic stability in individuals with metabolic syndrome. The relationship between EAT and the components of metabolic syndrome has been extensively studied from the perspectives of ectopic fat deposition, free fatty acid overload, low-grade inflammation, and oxidative stress to elucidate the core mechanisms underlying this condition. Lifestyle modifications, drug interventions, and surgical procedures can reduce EAT volume to improve metabolic syndrome outcomes and decrease cardiovascular events. This review provides a comprehensive summary of the correlation between EAT and metabolic syndrome, its influencing mechanisms, as well as treatment strategies.
